    The experiences of nurse educators in establishing a teaching practice in the care of older persons: A focused ethnography study

Nurse educators, who influence students' career decisions, lack expertise in older person care. The academic culture may affect educators developing gerontological expertise. Purpose: The study explored the culture of a Canadian pre-licensure nursing education program in relation to educators' expertise in gerontological nursing. Methods: In a focused ethnography, 22 nurse educators/researchers/administrators participated in interviews and/or observations conducted from March 2018 to December 2018. Content analysis of interview transcripts and fieldnotes occurred concurrently with data collection. Results: Themes characterizing the culture were: Structure and Hierarchy, Losing Gerontology, Teaching Challenges, and Valuing Older Persons and Their Care. Participants felt: a hierarchy limited gerontologists' support for undergraduate educators and the curriculum; the integrated curriculum reduced the focus on gerontology; limited professional development opportunities and excessive workload constrained building gerontology expertise; and valuing older persons and their care influenced access to gerontology resources. Conclusions: The culture of a pre-licensure nursing program impacted educators building expertise in older person care. More research is needed to overcome barriers that constrain educators' proficiency in gerontological nursing. Increasing the number of educators adept in gerontology will develop nurse graduates with an appreciation for working with older persons.

    查看更多>>摘要:? 2022The integrative review (IR) is a methodology of importance to nursing and other disciplines to evaluate and synthesize data from diverse sources to answer research questions, generate new theories, and to provide a comprehensive view of what is known regarding a topic of interest. Current, concise and introductory articles on how to conduct IRs are limited. Although a bias may exist that the IR is less rigorous than other types of literature reviews, the complexity of the search and evaluation of diverse sources increases the need for rigor to produce a well-written IR that advances evidence-based practice (EBP). This article provides a guide for students and beginning researchers to conduct this type of review by summarizing the IR into simple steps with strategies to increase rigor at each step. Use of an example IR related to acupuncture for neuropathy illustrates key points and lessons learned during the process.

    Preparing BSN students for a new workplace: Experiences in a COVID-19 designated unit as an RN extender

    查看更多>>摘要:? 2022 The AuthorsAcademic nursing programs were impacted by the onset of the pandemic with the challenges of social distancing, detecting, and controlling the spread of the virus. Many traditionally campus-based, face-to-face programs transitioned quickly to include virtual activities for classes, laboratories, and clinical experiences for students. Initially direct patient care clinical activities came to a halt due to concerns for student safety and overwhelming numbers of COVID-19 cases stressing health system resources. However, as COVID-19 cases began to rise and hospitals struggled with staffing shortages, it became necessary to explore how nursing students could contribute to relieving the staffing burden while obtaining critical education as RN Extenders. Students that participated in the RN Extender program were seniors entering the nursing workforce within the next two months. Significant student screening and preparation was provided to support students serving as RN Extenders in COVID-19 units. The prior working relationships and personal contact between academic and practice leaders, a sense of equal participation, clear and realistic expectations of the benefits and responsibilities of each group were crucial. Building ongoing and frequent opportunities for formal and informal communication among all stakeholders was vital and the major contributor to the success of this program.

    Dissemination enhancement in Doctor of Nursing Practice students

    查看更多>>摘要:? 2022 Elsevier Inc.Purpose: Students in Doctor of Nursing Practice (DNP) programs graduate with a completed DNP Project focused on practice improvement utilizing evidence-based research. Despite the value in disseminating the DNP Project outcomes to an audience of interest, many DNP graduates do not pursue dissemination beyond conclusion of their project and graduation. Methods: The DNP Project didactic course and the DNP Project manuscripts were revised with active learning assignments designed to improve knowledge, skills, and confidence in professional writing and dissemination. A cohort of 26 students was invited to participate in informal focus groups to determine impact of the revisions. The program director tracked the number of students accepted for conference presentations or manuscripts post-implementation. Results: Of those invited to the focus groups, 25 students provided feedback. The students reported a perceived increase in competence and confidence in their ability to submit a professional abstract or manuscript for presentation or publication. Faculty noted an increase in the number of students who proceeded with dissemination of their DNP Project. Conclusion: Capturing the momentum for dissemination by revising the DNP Project didactic course and DNP Project manuscripts proved valuable for enhancing graduates disseminating professionally thus contributing to the translation of evidence-based practice into healthcare.

    Integrating a simulated electronic medical record system and barcode medication administration into a pre-licensure nursing program

Nursing schools are exploring implementing similar technology to promote best practice. Purpose: Our institution developed a quality improvement project to evaluate the integration of similar technology into pre-licensure curriculum. Method: Our quality improvement project examined using a simulated electronic medical record (EMR) and a barcode medication administration (BCMA) system in pre-licensure nursing curriculum. In our initial project, 96 second-semester BSN students participated in a skill check off using the system. Results: Student perception was positive, 96% of respondents stated the system increased realism, and 94% indicated improved clinical preparedness. Conclusion: Implementing this system was a positive experience which enriched our pre-licensure curriculum.

    Commitment, job satisfaction and personality: A cross sectional study of generational cohorts in nursing students

Purpose: The purpose of this study was to investigate generational differences in organizational commitment, job satisfaction and personality traits in a sample of nursing students preparing for the workforce. Methods: This cross-sectional study used data collected from pre-licensure final semester nursing students and degree completion students (n = 313) from three different schools about job satisfaction, personality traits and organizational commitment. Results: Findings showed the scores between the generations were not significantly different for job satisfaction (p = 0.86) and organizational commitment (p = 0.40). Significant differences were found between the generations for the personality measures of agreeableness (p = 0.02), conscientiousness (p = 0.01), and emotional stability (p = 0.00). Discussion/conclusions: Understanding generational differences in nursing students entering the workforce can serve academia and practice partners to focus curriculum and resources appropriately to prepare the next generation of nurses for the challenges in a time of crisis.

    Managing the unexpected: Stressors and solutions for challenges experienced by RN-BSN students during an unprecedented global pandemic

A large contingent of registered nurses (RNs), working as essential workers, were also attending classes in online accelerated programs (OAPs) to earn their BSN degree. Attending school as a working RN, can be stressful. During an active pandemic, it can be overwhelming. Purpose: This quality improvement project was aimed to identify students' stressors in an online accelerated RN-BSN program, and to use the results to make adjustments in coursework to decrease academic stress while improving student success. Method: A Qualtrics survey was distributed to 1992 active RN-BSN students to identify the student's “biggest challenges or stressors”, and 302 students responded listing 1105 stress responses and elaborations. The survey included some academic suggestions by faculty to alleviate course stress. Students commented on the usefulness of suggestions and proposed additional ideas. Compassionate interventions were employed in each core nursing course. Results: After 2 terms a post-intervention survey was conducted to evaluate the effectiveness of the course interventions employed. A total of 276 students participated in the post-intervention survey, with 778 responses to the employed interventions. Conclusions: Findings show the importance of faculty vigilance in assessing cause for declines in academic performance. Using an inquiring approach and flexibility to adapt coursework while maintaining student learning outcomes is possible and can play a significant role in student success.

    Postgraduate and undergraduate student nurses' well-being: A scoping review

This scoping review article aimed to explore the well-being among student nurses, its antecedence and consequences, as well as interventions that could help maintain and improve the well-being. PRISMA-ScR was used as a guideline, and a literature search was retrieved from four databases: Scopus, Web of Science, CINAHL, and PubMed. Fifty-four of 1104 articles were included and analyzed in this review. The review results indicated that the internal factors of well-being include the student's personality, psychological perceptions, sociodemographic, and health conditions. In contrast, the external factors were university location, facilities, graduate program, services, support, and learning environment. In addition, physical activity is considered a major intervention to improve well being. Therefore, this review serves as an input for universities, nursing schools, and nurse educators to pay attention to internal and external factors as well as to provide a physical activity intervention and good learning environment that make the students feel confident to achieve academic goals as a foundation to good nursing practice.

    Diving in: Using a “Shark Tank” approach to teach business skills to future DNP leaders

Advanced clinical knowledge, leadership skills, and the development of quality/safety competencies uniquely prepare the DNP program graduates to drive organizational change. Adding business and financial competencies to the skill set of DNP graduates strengthens the impact and value of their role on financial, quality, and operational outcomes. The Organizational Systems and Healthcare Financing course in a DNP program was redesigned to engage learners using an innovative approach to teach business and financial principles. This paper aims to (a) describe a novel “Shark Tank” approach whereby students develop and “pitch” their business proposals to a panel of healthcare executives; (b) share examples of impactful change projects by student teams; (c) report DNP course and program evaluations including students' satisfaction and perceptions of value and knowledge gained in business principles; and (d) report opportunities for bidirectional mentorship, faculty recruitment, and succession planning. The success of this innovative team-based approach for teaching business/financial skills better prepares future DNP leaders and has implications for other DNP programs. Using this teaching strategy created opportunities for faculty recruitment, succession planning, and bidirectional mentorship of DNP-prepared nurse leaders.
