    Comparison of Permethrin-Based Treatment Strategies against Scabies in Infants and Young Children

    查看更多>>摘要:Objective To optimize the permethrin-based therapies for scabies infestations in infants and young children, the efficacies of 3 different regimens were evaluated. Study design The retrospective analysis encompassed 85 infants and children aged <4 years with scabies. The children had received either topical permethrin 5% on the entire body on days 1/8, on days 1/8/15, on days 1/8/15 plus interim applications restricted to hands and feet on days 2/3/4//9/10/11, or alternative treatments. Results The intensified regimen, consisting of full-body permethrin applied on days 1/8/15 and hands/feet on days 2/3/4/19/10/11, resulted in cure of scabies in 73.5% of the cases. The cure rates were significantly greater compared with full-body permethrin given on days 1/8, which led to eradication in 44%, and were greater compared with the clearance in children who had received full-body permethrin on days 1/8/15 (53.8%) or alternative treatments (60%). For patients in whom permethrin had previously been applied, the intensified regimen resulted in eradication of scabies in 71.4% of the cases, compared with 30% and 55.6% after full-body permethrin on days 1/8 and 1/8/15, respectively. Conclusions The intensified regimen of full-body permethrin plus interim applications on hands/feet, which aims at reducing the number of mites present on the frequently heavily infested palmoplantar sites in addition to the standard entire body application, appears efficacious in curing scabies in young children.

    Cytogenetic Patterns, Congenital Heart Disease, and Thyroid Dysfunction in Children with Down Syndrome

    查看更多>>摘要:Objectives To study the cytogenetic patterns, congenital heart disease, and thyroid dysfunction in children with Down syndrome. Study design This was a retrospective observational study of children with Down syndrome over a period of 20 years, from a major referral institution in Kerala state, South India. The cytogenetic patterns, echocardiography reports, and thyroid profiles were analyzed using SPSS, version 20, software. The prevalence of heart disease and thyroid status in the various cytogenetic patterns also were analyzed. Results The prevalence of translocation (9.45%) was high compared with the reported 4% in the literature. More of the younger mothers had translocation with a greater, but not statically significant, incidence of heart disease. Mosaic karotypes (3.04%) were also greater than reported (1%) in the literature, with female preponderance. Heart disease was seen in 58% of cases, with atrial septal defect being the most common lesion, compared with atrioventricular septal defect noted in literature. Hypothyroidism was noted in 31.2% with no difference among the cytogenetic groups. There was no case of hyperthyroidism. Conclusions The high prevalence of translocation and mosaic Down syndrome stresses the need for routine karyotyping in children with Down syndrome. The need for routine screening and regular follow up of heart diseases and thyroid status should be emphasized.

Our retrospective study suggests the need for combined hemoglobin and serum ferritin iron deficiency screening and raising the diagnostic serum ferritin threshold to 24-25 mu g/L.

    查看更多>>摘要:As previously fatal diseases of childhood evolve into chronic illnesses of adolescence and young adulthood, some children with palliative care needs develop chronic pain. Many pediatric palliative care teams specialize in pain management, which focuses on improving quality of life by reducing immediate physical suffering, with less expertise in or emphasis on chronic long-term pain management. A child presented for care by a palliative care team in a large quaternary care children's hospital suffered from chronic pain. She was treated with a continuous intravenous (IV) opioid infusion and had a life expectancy of several years or more. With increased awareness of the risks of long-term opioid use, and informed by the best-available evidence for chronic pediatric pain management, we sought an opioidsparing approach in managing the child's pain. This led to challenges in maintaining the therapeutic alliance with the family. For the pediatric palliative care prescriber, appropriate management of chronic pain conditions poses a dilemma for safe prescribing in children with life-limiting diagnoses, but in whom prognosis can be unknowable and goals fluctuating. Balancing a family's fear of chronic suffering vs long-term risks of opioids in children can be complex, and despite best efforts at high-quality communication, can lead to conflict between families, prescribers, and other medical care teams. More coordination is needed between the fields of pediatric chronic pain management and pediatric palliative care, in order to meet best the needs of children who are eligible for care by both.

Although all patients showed rapid clinical improvement, many had persistent cardiac magnetic resonance imaging findings at 3- to 8-month follow-up.
