    Risk factors for Brucellosis and knowledge-attitude practice among pastoralists in Afar and Somali regions of Ethiopia

    查看更多>>摘要:ABS T R A C T Background: Brucellosis is a neglected bacterial zoonotic disease with substantial economic impact on house-holds. Pastoral communities are a potential risk group due to their way of life being closely interlinked with their large livestock herds. Methodology: A semi-structured questionnaire survey was conducted in households in the pastoral Afar and Somali (SRS) regions. All households had people and animals serologically tested for brucellosis. Questions were related to husbandry, consumption habits, and knowledge-attitude-practice towards the disease and zoonoses. Descriptive statistics and logistic analysis were performed to assess potential risk factors for having households with positive humans and/or animals. Result: 647 households were included in the survey. Herd brucellosis prevalence was 40.3 % (15.9-86.3 % in Afar; 4-72.2 % in SRS). Over half (56.3 %) of the households in Afar and 41.8 % in SRS had at least one human reactor. Nearly a quarter of the households (22.8 %), recalled abortions in goats in the last 12 months, whereas 52.5 % and 50.3 % recalled stillborn in all species and membrane retentions respectively. All respondents drank raw milk and discarded animal afterbirths in the direct surroundings with minimal protection. Risk factors for animal reactors were goat herd size, and goat abortion. There was no identified risk factor for having human reactors in households. None of the households knew about brucellosis. Conclusion: Although being endemic in Afar and SRS, Brucellosis is not known by the pastoralists. Brucellosis control programs will have to be tailored to the pastoral context, accounting for their mobility, large, multi-species herds and habits.

    Flow of information for bovine health surveillance: Two socio-economic models demonstrating the impact of the organisational profiles of local actors

    查看更多>>摘要:Passive surveillance is based on spontaneous reporting to veterinary authorities of disease suspicions by farmers and other stakeholders in animal production. Stakeholders are considered "actors" in sociology of organisations research. In veterinary public health, passive surveillance is considered to be the most effective method to detect disease outbreaks and to generate epidemiological information for decision-making on surveillance and control strategies. Nevertheless, under-reporting of cases is an inherent problem, reducing the ability of the system to rapidly detect infected animals. Previous studies have shown, for example, that passive surveillance for bovine brucellosis in France, through compulsory reporting of all bovine abortions, has limited sensitivity, with variability in reporting rates despite similar cattle farming profiles. Based on this observation and on sociological literature in health surveillance, we hypothesised that oversight organisational factors in different areas influence health actor contributions to passive surveillance. Therefore, to improve the efficiency of surveillance systems, we need to understand the organisational levers (supporting factors) and organisational drags (hindering factors) on the production and dissemination of health information. We conducted semi-structured interviews with the surveillance actors in two administrative geographic divisions in France (Departments A and B) with similar cattle farming profiles but contrasting abortion reporting rates (low and high, respectively). We assumed that these rates were related to health actor organisation in each administrative division. We mapped actor relationships and looked for behavioural recurrences and differences between the two departments. This analysis led to two socio-economic models explaining the configurations observed: pro-curative in Department A, and pro-preventive in Department B. These models showed a link between the level of competition endured by veterinarians on the sale of veterinary medicinal products and the overall contribution of the actors to health surveillance. The pro-preventive model had a higher contribution to surveillance than the pro-curative model. Importantly, the nature of the information produced in this configuration of actors corresponded to the needs of surveillance, providing collective and early information that circulated more readily between actors. We highlighted three characteristics that help to identify the configuration of a system of actors: 1) the pressure of competition exerted on veterinarian activities; 2) the dominant business model and form of organisation of veterinary clinics; and 3) the frequency of interactions between the main surveillance actors outside of crises. The first two characteristics affect the local contribution to data reporting for surveillance, and the third affects network responsiveness in a health crisis.

    Simulating the mechanics behind sub-optimal mobility and the associated economic losses in dairy production

    查看更多>>摘要:Hoof disorders and sub-optimal mobility (SOM) are economically important health issues in dairy farming. Although the dynamics of hoof disorders have an important effect on cow mobility, they have not been considered in previous simulation models that estimate the economic loss of SOM. Furthermore, these models do not consider the varying severities of SOM. The objective of this study was to develop a novel bio-economic simulation model to simulate the dynamics of 8 hoof disorders: digital dermatitis (DD), interdigital hyperplasia (HYP), interdigital dermatitis/heel-horn erosion (IDHE), interdigital phlegmon (IP), overgrown hoof (OH), sole haemorrhage (SH), sole ulcer (SU) and white-line disease (WLD), their role in SOM, and estimate the economic loss of SOM in a herd of 125 dairy cows. A Reed-Frost model was used for DD and a Greenwood model for the other 7 hoof disorders. Economic analysis was conducted per mobility score according to a 5-point mobility scoring method (1 = perfect mobility; 5 = severely impaired mobility) by comparing a scenario with SOM and one without SOM. Parameters used in the model were based on literature and expert opinion and deemed credible during model validation rounds. Results showed that the mean cumulative incidence for maximum mobility scores 2-5 SOM episodes were respectively 34, 16, 7 and <1 episodes per 100 cows per pasture period and 39, 19, 8, <1 episodes per 100 cows per housing period. The mean total annual economic loss due to SOM resulting from the hoof disorders under study was (sic)15,342: (sic)122 per cow per year. The economic analysis uncovered direct economic losses that could be directly linked to SOM episodes and indirect economic losses that could not be directly linked to SOM episodes but arose due to the presence of SOM. The mean total annual direct economic loss for maximum mobility score 2-5 SOM episodes was (sic)1129, (sic)3098, (sic)4354 and (sic)480, respectively. The mean total annual indirect economic loss varied considerably between the 5th and 95th percentiles: (sic) - 6174 and (sic)19,499, and had a mean of (sic)6281. This loss was composed of additional indirect culling due to SOM (similar to 65%) and changes in the overall herd milk production (similar to 35%) because of additional younger replacement heifers entering the herd due to increased culling rates. The bio-economic model presented novel results with respect to indirect economic losses arising due to SOM. The results can be used to stimulate farmer awareness and promote better SOM management.

    A stochastic modelling approach to determine the effect of diverse Staphylococcus aureus strains on the economic and epidemiological outcomes of mastitis intervention strategies in dairy cattle

    查看更多>>摘要:Staphylococcus aureus (S. aureus) strains with considerable genetic and phenotypic differences have previously been identified. The economic and epidemiologic impact of S. aureus mastitis has been investigated, but none of these studies took differences between strains into account. Here we aimed to investigate how differences between S. aureus strains affect the economic and epidemiologic outcome of various intervention strategies against clinical and subclinical intramammary infections. Five S. aureus strains were modelled using a stochastic bioeconomic model simulating a dairy herd of 200 cows using single-day time steps. The strain characteristics of the five simulated S. aureus strains (general, contagious, spill-over, clinical and persistent) were based on divergent phenotypes as described in literature. Outcomes of the model included incidence (both clinical and subclinical), number of antibiotic treatment days, number of culled cows, and net income. Intervention strategies against clinical and subclinical intramammary infections were based on (variations of) intramammary antibiotic treatment, testing, and culling. Both single and multiple pathogen (intramammary infection caused by S. aureus, Escherichia coli, and non-aureus staphylococci) scenarios were simulated to determine the effect of the five S. aureus strains on the impact of 19 different intervention strategies. The results showed that the incidence (both clinical and subclinical), number of treatment days, number of culled cows, and net income varied considerably for the different S. aureus strains. Comparison of the model outcomes within and between strains showed that for most intervention strategies the relative impact differed per strain. However, the intervention strategy with the best outcome for most variables and strains was the culling of cows with a recovery probability lower than 50%. This shows that the relative economic and epidemiologic impact of most of the modelled intervention strategies were strain-dependent, while some intervention strategies were not strain-dependent. From this, we conclude that, depending on the intervention strategy applied on a farm, it could be advantageous to type S. aureus to determine whether it would be economically and epidemiologically beneficial for the existing intervention strategy to be changed.
