    Bailout Strategies for Abrupt Change in Woven Endobridge 17 Device Orientation After Detachments: Technical Note of 2 Anterior Communicating Artery Aneurysm Cases

    查看更多>>摘要:? 2022 Elsevier Inc.Background: Little information is available regarding technical challenges with the new lower profile Woven EndoBridge (WEB 17) system intended for smaller aneurysms. Methods: We report illustrative cases of technical complications encountered with 2 anterior communicating artery aneurysms treated by the WEB 17 system requiring rescue stenting in both cases, discussing technical nuances regarding potential reasons for the encountered failures along with management plan. Results: Over a span of 1 year (January 2021 to January 2022), 45 WEB embolization procedures were performed at 2 institutions. Two procedures were complicated by abrupt change in orientation of the WEB device immediately after detachment from the delivery wire. In the first case, abrupt angulation with subsequent migration and prolapse out of the aneurysm sac into the distal right anterior cerebral artery was encountered with unsuccessful retrieval despite multiple attempts using a variety of devices, eventually requiring rescue stenting. A similar sudden orientation change was noted in the second case with partial prolapse from the aneurysm sac similarly bailed out by intracranial stenting. Both patients recovered to preprocedural baseline with no permanent deficits and eventually were discharged home. Conclusions: Intrasaccular WEB 17 embolization may be technically challenging in smaller wide-necked aneurysms with acute aneurysm–parent artery angulation with abrupt changing of WEB device orientation after detachments with device migration and prolapse into the parent vessel requiring rescue stenting. Proper WEB 17 device sizing and vigilance in the transition phase between the end of deployment and detachment windows of the procedure are paramount to treatment success. Routine use of antiplatelets in cases of anatomical aneurysms that are anticipated to be challenging might be a useful strategy if bailout stenting is needed.

    查看更多>>摘要:? 2022 Elsevier Inc.Cranial nerve schwannomas accounts for around 8% of all benign intracranial tumors, arising most commonly from the vestibular nerve, followed by the trigeminal nerve and other lower cranial nerves. However, trochlear schwannoma in a patient without neurofibromatosis-2 are extremely rare and to date, fewer than 100 cases have been reported in the literature. They are either asymptomatic or present with ophthalmologic or neurologic symptoms. Diplopia is the most common initial symptom. As the tumor grows, it can compress the surrounding brainstem and other cranial nerves, causing neurologic symptoms. Asymptomatic lesions are detected incidentally following imaging for some other reason. There are no clear guidelines for the management of these tumors. In general, small asymptomatic tumors are closely observed by serial imaging and symptomatic or larger tumors are managed by surgical excision and/or stereotactic radiosurgery.1-7 Here we present a 41-year-old female patient with incidentally detected left trochlear schwannoma during the follow-up magnetic resonance imaging (MRI) scans. She was followed up regularly with multiple repeat MRI. Recently she started complaining of occasional headaches, and MRI showed a left peimesencephalic cistern tumor causing mass effect on the ipsilateral midbrain. There was also significant brainstem edema. Hence she underwent left retromastoid suboccipital craniectomy, lateral supracerbellar approach, and complete excision of the tumor. Postoperatively the patient had an uneventful recovery without any new neurologic deficits. At 6 months’ follow-up the patient is doing well.

    Infantile Choroid Plexus Papilloma with Multiple Peritumoral Cysts

    查看更多>>摘要:? 2022 Elsevier Inc.Infantile choroid plexus papilloma (CPP) associated with multiple peritumoral cysts is a rare variant of CPP, and clinical course and optimal management are largely unknown. A 9-month-old boy presented with a large solid tumor in the left lateral ventricle associated with multiple peritumoral cysts, arachnoid cysts, and hydrocephalus containing xanthochromic fluid with high protein content. Shrinkage of these cysts and resolution of hydrocephalus were achieved after total resection of the hypervascular solid part of the tumor. Histological examination confirmed the solid part of the tumor as CPP and showed that the wall of the peritumoral cysts consisted of reactive gliosis without neoplastic cells. Follow-up magnetic resonance imaging 12 months after surgery revealed that these cysts remained stable. CPP with nonenhancing peritumoral cysts can be managed by resection of only the solid part of the tumor without permanent cerebrospinal fluid diversion.

    Fusiform Dilatation of the Internal Carotid Artery in Childhood-Onset Craniopharyngioma: A Systematic Review

    查看更多>>摘要:? 2021Objective: Patients with fusiform dilatation of the carotid artery (FDCA) following pediatric craniopharyngioma resection typically have a benign clinical course. We reviewed the neurosurgical literature for FDCA outcomes after resection of these tumors. Methods: Using PubMed, Web of Science, and Cochrane databases, we identified surgical series or case reports reporting incidences of FDCA following craniopharyngioma resection. Inclusion criteria included FDCA outcomes reported specifically after craniopharyngioma resection, with at least 6 months of follow-up data. Results: Our literature search yielded 15 full-text articles comprising 799 patients (376 [52.3%] males). The weighted mean follow-up was 74.8 months (range, 9–140 months). Most tumors were suprasellar (62.1%), with traditional microsurgery being more commonly employed than endoscopic endonasal surgery (80.9% vs. 19.1%). Gross total resection was achieved in 42.6% of cases. There were 55 aneurysms reported, most commonly occurring at the terminal internal carotid artery (66.7%). Aneurysmal progression on follow-up occurred in 10 (18.5%) cases, with no reports of rupture. Ten (18.2%) aneurysms were treated with clipping, endovascular, or bypass techniques. Conclusions: FDCA is a rare complication following pediatric craniopharyngioma resection. The exact cause is unclear, and factors related to tumor invasiveness, size, location, and differences in surgical approach may contribute to FDCA development. Most patients who go on to develop FDCA have an innocuous course on follow-up, with no reports of rupture in the present literature. For this reason, patients rarely require surgical or endovascular intervention for these lesions, and conservative management is favored.

    Endonasal Endoscopic Approach for Minimally Invasive Orbital Decompression in Nonthyroid Proptosis—A Scoping Review

Since then, this approach also has been used to treat other causes of proptosis. This review systematically identifies surgical outcome and complication rates in patients without thyroid proptosis who underwent endoscopic endonasal orbital decompression. Methods: Databases were searched using the following search terms: orbital disease, surgical decompression, and endoscopic endonasal approach. Two independent reviewers screened all abstracts and titles for relevance and all articles passing this screen were subjected to full-text review. To assess risk of bias, we used ROBINS-I (Risk Of Bias in Non-randomized Studies–of Interventions). Results: Eight studies with a total of 74 patients with nonthyroid proptosis were included. Pre- and postoperative eye examination was performed in all studies, but the extent of examination was varying. With a mean age of 35.7 years, most patients were adolescent, and most pathologies induced unilateral proptosis Complications to EEA for orbital decompression were transient diplopia (5 patients/6.8%), transient facial dysesthesia (2 patients/2.7%), ptosis (1 patient/1.4%), infarction (1 patient/1.4%), sinus obstruction (1 patient/1.4%), and enophtalmos (1 patient/1.4%). The authors reported successful reduction of proptosis in all but 2 patients (97.2%), and only 2 authors reported a need for secondary decompression. Conclusions: Medial orbital decompression using EEA is a feasible approach for orbital decompression in patients with nonthyroid proptosis. While being comparable in primary outcome to transorbital approaches, the EEA seems superior in terms of complication rates

    Technological Improvements in Low- and Middle-Income Countries (LMICs): A Review of the Literature and the “Sociedad de Lucha Contra el Cáncer” (SOLCA) Institutional Experience in Neuro-Radiosurgery During the Coronavirus Disease 2019 (COVID-19) Pandemic

Our goal was to report our experience with the replacement and implementation of linear accelerators with radiosurgery capabilities at “Sociedad de Lucha Contra el Cáncer” (SOLCA) Hospital in Quito, Ecuador, and give some recommendations for future technological replacements (TRs). Methods: Two surveys were performed in SOLCA's radiosurgery department, one before the TR was finalized and one after, consisting of an open and multiple-choice questionnaire. Questions focused on the performance of the new equipment, perceptions regarding the training, and the influence of the pandemic on the whole process. In addition, we share our experience regarding the difficulties and benefits of TR at SOLCA. Results: The most-reported limitation was lack of training (48%). By the time of the second survey, 95.2% of the staff had already treated patients or planned a radiosurgical procedure; 42.9% considered training to have been adequate, and 76.2% felt that the pandemic hindered the training process. Currently, 33 radiosurgeries have been done (26 for the central nervous system and 7 stereotactic radiation body therapies). Conclusions: The TR in SOLCA had similarities with other experiences in low- and middle-income countries, but the pandemic brought additional limitations, mainly complicating the staff training. Nevertheless, those limitations can be resolved with a structured training program and international collaboration. Overall, the benefits obtained from a TR result in exponentially better medical care and accessibility to novel treatments.

    Hypertonic Saline Treatment in Traumatic Brain Injury: A Systematic Review

    查看更多>>摘要:? 2022Hypertonic saline (HTS) is a widely used adjunct in the treatment of traumatic brain injury (TBI). However, there is significant variability in practice patterns. Toward the goal of optimality and standardization in the use of HTS in TBI, we performed a comprehensive review of clinical protocols reported in the neurosurgical and neurocritical care literature. PubMed, Web of Science, Cochrane, Scopus, and Embase were independently queried between October and November 2021. The PRISMA guidelines were used throughout the screening process. We identified 15 high-quality studies representing data from 535 patients. We extracted patient demographics, Glasgow Coma Scale (GCS) score, mechanism of injury, HTS dosage, and rate of administration. Various HTS concentrations including 3%, 5%, 7.2%, 7.5%, and 20% were used. Modes of HTS administration included bolus (n = 125) and infusion (n = 376). Average length of stay was 22.4 days. Patient GCS score on initiation of HTS was depressed (average mean, 7.15; average median, 4.25 for studies reporting mean and median GCS, respectively). Excluding 2 studies with ambiguous doses, the mean HTS dosage was 2.7 × 102 mL across 8 studies and 2.5 mL/kg across 5 (with average post-HTS osmolality level of 304.6 mOsm/L reported in 3 studies). Infusions of 3% and 7.5% HTS are the most used concentrations given their efficacy in reducing intracranial pressure (ICP) and improving GCS score. In addition, lower HTS concentrations strongly correlated with greater ICP reduction. Therefore, lower concentrations of HTS may be practical therapeutic agents for patients with TBI given their efficacy in ICP reduction and safer complication profile compared with greater HTS concentrations. Evidence-based parametric use of HTS stands to improve patient outcomes by standardization of varied clinical practice.

    Diversity in Neurosurgical Recruitment and Training in the United States: A Systematic Review

    查看更多>>摘要:? 2022 The AuthorsObjective: The benefits of diversity are particularly salient in neurosurgical training because of treatment of varied patient populations and the importance of close collaboration between trainees and attending neurosurgeons of different backgrounds. However, there is a paucity of literature that comprehensively examines diversity in neurosurgical recruitment and training. The aim of this study is to systematically review the scope of diversity in neurosurgical recruitment and training. Methods: PudMed, Embase, and Scopus were searched since inception to October 31, 2021. Inclusion consisted of 1) on neurosurgery training; 2) sample was medical student, resident, fellows or attending; 3) discussed diversity variable(s). This study followed PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines. Results: Of 1578 articles, 15 studies were included. Studies largely reported gender (n = 14) underrepresentation in neurosurgery compared with racial/ethnic diversity (n = 2) and socioeconomic status (n = 1). From studies focusing on residency match (n = 4), women medical students placed greater emphasis on mentorship, earlier clinical exposure, and program camaraderie and reputation as most important. From studies (n = 4) reporting retention and attrition, although women residents and attendings representation has increased, black, indigenous and people of color groups continue to represent <15% of studies on neurosurgical training. Studies (n = 4) discussing research suggested that women residents were more likely to have lower h-indices with fewer research grants. Studies (n = 4) describing workforce employment further showed that women and racial/ethnic minority groups were less likely to hold high academic and leadership positions. In studies (n = 4) on work–life balance, 18.5% women attendings found less career fulfillment and 20.4% would not choose to pursue neurosurgery again because of lack of protected personal time. Conclusions: This systematic review shows the scope of studies of diversity within neurosurgery and provides impetus for efforts to expand our understanding of diversity within the field.

    Perioperative Pain Management for Elective Spine Surgery: Opioid Use and Multimodal Strategies

The lack of well-studied alternative treatment regimens has led to a substantial burden of opioid addiction in the United States. These forces have led to a huge economic burden on the country. The spine surgery population is particularly high risk for uncontrolled perioperative pain, because most patients experience chronic pain preoperatively and many patients continue to experience pain postoperatively. Overall, there is a large incentive to better understand comprehensive multimodal pain management regimens, particularly in the spine surgery patient population. The goal of this review is to explore trends in pain symptoms in spine surgery patients, overview the best practices in pain medications and management, and provide a concise multimodal and behavioral treatment algorithm for pain management, which has since been adopted by a high-volume tertiary academic medical center.

    MRI–Negative Cushing's Disease: A Review on Therapeutic Management

    查看更多>>摘要:? 2022 Elsevier Inc.Objectives: In this systematic review and meta-analysis, we review the literature regarding patients with Cushing's disease (CD) with negative or inconclusive magnetic resonance imaging (MRI). Methods: A quantitative systematic review was performed. Article selection was performed by searching MEDLINE (using PubMed), EMBASE, and Cochrane electronic bibliographic databases. Results: 28 articles described surgical management of inconclusive MRI or MRI-negative CD. A total of 858 patients underwent surgery for their Cushing adenoma. Different types of surgery, including endoscopic endonasal transsphenoidal surgery (EETS) (190 cases) and microscopic endonasal transsphenoidal surgery (METS) (488 cases), were performed on patients with MRI-negative CD. 7 studies, which included 164 patients, did not describe any surgery. EETS and METS are conducted to achieve selective adenomectomy (231 cases), partial adenomectomy (80 cases), total adenomectomy (13 cases), hemihypophysectomy (15 cases), or enlarged adenomectomy (48 cases). Based on available data on these studies, the remission rate, persistence rate, and recurrence rate after different types of surgeries on patients with MRI-negative CD were 72.97%, 27.03%, and 12.05%, respectively. There was no statistically significant difference between EETS and METS in the subanalysis regarding recurrence rate, remission rate, and persistence rate. However, the recurrence rate in the METS group is almost 3 times higher than in the EETS group. Conclusions: Surgery has a good prognosis in patients with MRI-negative CD in terms of remission, and EETS has a lower rate of disease recurrence than METS; therefore, EETS seems to be the potential recommended treatment technique, while to confirm the therapeutic method of choice, further investigations should be done.
