查看更多>>摘要:Disease of the anal sac is a common reason for presentation of dogs (and less commonly cats) to the veterinary surgeon. They are often just emptied by a veterinary surgeon or veterinary nurse; but this does not always lead to resolution of clinical signs. An understanding of the diseases affecting the anal sacs and possible treatment options can increase the likelihood of a positive outcome. This paper will look at these issues, together with mention of perianal fistula (anal furunculosis) a diseasethat often involves the anal sacs.

This was the first indication of a problem, however inadvertent endobronchial intubation initially, and accidental extubation, had occurred in the period immediately prior to surgery. The presence of the pneumoretroperitoneum alerted the clinicians to the likelihood of airway trauma, and mechanical ventilation was discontinued immediately. After completion of the coeliotomy, surgical exploration of the cervical trachea failed to detect an obvious tracheal tear. Subsequent chest radiographs confirmed the presence of pneumoretroperitoneum, pneumomediastinum, mild pneumothorax and subcutaneous emphysema in the cervical region. No further diagnostics were performed, thus the site of the tear could not be definitively diagnosed but was suspected to be distal to the thoracic inlet. The cat recovered uneventfully and was discharged three days later after conservative management. This report highlights the ability to rapidly recognise acute airway trauma by the presence of pneumoretroperitoneum, prompting immediate cessation of positive-pressure ventilation which, if continued, could have exacerbated the pneumothorax in th ISCase.

Many affected cats have a history of persistent or retained deciduous teeth, and in some cats their permanent teeth also do not erupt properly. The patellar fractures are usually unilateral at first presentation but become bilateral within 3-9 months. Pin and tension band wire fixation is associated with a high complication rate and often results in further fragmentation of the patella. A technique that does not involve inserting an implant into the sclerotic patella, such as circumferential wiring, tension band without a pin, or a tibial quadriceps repair may be beneficial, but conservative treatment can yield equally good results and is inevitably associated with a lower complication rate. Further work is needed before definitive statements can be made regarding the best treatment for this condition, as there are too low numbers or too many variables in the published retrospective studies. Many affected cats will fracture other bones as they age. An underlying bone or connective tissue disease is suspected in cats with patellar fractures and persistent deciduous teeth, and for ease of discussion and case recruitment the disease has been termed knees and teethsyndrome or KaTS.

Rationalising and optimising their use in veterinary medicine is imperative. Antimicrobial stewardship is a key strategy used to limit the development of resistance, as it provides for careful and responsible management of antimicrobial drugs. It could form an integral part of a veterinary practice's policyon antimicrobial use. Development of an antimicrobial stewardship plan requires detailed knowledge of antimicrobial drugs, infectious agents, and their likely resistance patterns, plus close collaboration with a microbiology service, and it can be informed by an antibiogram. This article discusses AMR, antibiosis and antimicrobial stewardship programmes.

While dermatophytosis is generally considered to be a mild and self-limiting disease, there is a need to manage it more aggressively in charity-owned cats than when presented in client-owned cats. This article considers the implications of ringworm in rescue shelters, and discusses diagnostic, treatment and homing approaches in this context.
查看更多>>摘要:Skin grafting involves harvesting of a free segment of skin from a donor site and transplanting it to a distant recipient site, usually one outside the reach of skin flaps. It is most commonly used to cover large defects of the distal limb, where a limited amount of local skin is available for reconstruction. Less commonly it may be used to cover large defects on the trunk, for example caused by burns. Grafts are completely detached from any blood supply following harvesting, and must re-establish vascular connections with the recipient site in order to survive. Understanding the process of engraftment or graft 'take' is essential to minimise the risk of postoperative complications. This article will review the general principles of skin grafting, the most commonly performed techniques, and the steps required to minimise complications.

Certainly, the widely accepted practice ofkeeping them outdoors year-round in what is a totally unsuitable climate contributes massively. The pre- and post-hibernation periods are times of high morbidity and mortality in tortoises. It is important for veterinary professionals not only to recognise how to prevent such problems by advising on appropriate care, but also to treat such issues successfully in the clinic environment. The following article discusses the major failures in captive care that contribute to peri-hibernation problems in tortoises, as well as outlining their diagnosis and treatment.

The study recruited 360 dogs in order to answer the question 'is administration of pimobendan to dogs with degenerativemitral valve disease prior to the onset of heart failure beneficial?' The main conclusions of the study have just been published and the answer is 'Yes'.
