    "A diamond-shaped"penoplasty technique with or without concurrent suprapubic liposuction for adult-acquired buried penis:clinical outcomes and patient satisfaction rates

    查看更多>>摘要:Various techniques have been described for reconstructing the skin of the penile shaft;however,no universally accepted standard exists for correcting buried penis in adults.We aimed to describe a new technique for correcting an adult-acquired buried penis through a diamond-shaped incision at the penopubic junction.We retrospectively analyzed data from patients treated with our technique between March 2019 and June 2023 in the Department of Andrology,Nanjing Drum Tower Hospital(Nanjing,China).Forty-two adult males with buried penises,with a mean(±standard deviation[s.d.])age of 26.6(±6.6)years,underwent surgery.All patients were obese,with an average(±s.d.)body mass index of 35.56(±3.23)kg m-2.In addition to phalloplasty,32 patients concurrently underwent circumcision,and 28 underwent suprapubic liposuction.The mean(±s.d.)duration of the operation was 98.02(±13.28)min.The mean(±s.d.)duration of follow-up was 6.71(±3.43)months.The length in the flaccid unstretched state postoperatively was significantly greater than that preoperatively(mean±s.d:5.55±1.19 cm vs 1.94±0.59 cm,P<0.01).Only minor complications,such as wound disruption(7.1%)and infection(4.8%),were observed.The mean(±s.d.)score of patient satisfaction was 4.02(±0.84)on a scale of 5.This technique provides excellent cosmetic and functional outcomes with a minimal risk of complications.However,additional clinical studies are needed to evaluate the long-term effects of this procedure.

    Sperm RNA quantity and PRM1,PRM2,and TH2B transcript levels reflect sperm characteristics and early embryonic development

    查看更多>>摘要:Spermatozoa have a highly complex RNA profile.Several of these transcripts are suggested as biomarkers for male infertility and contribute to early development.To analyze the differences between sperm RNA quantity and expression of protamine(PRM1 and PRM2)and testis-specific histone 2B(TH2B)genes,spermatozoa from 33 patients who enrolled in assisted reproduction treatment(ART)program were analyzed.Sperm RNA of teratozoospermic(T),oligoteratozoospermic(OT),and normozoospermic(N)samples was extracted,and the differences in transcript levels among the study groups were analyzed by quantitative real-time polymerase chain reaction(qRT-PCR).The correlations of total RNA per spermatozoon and the expression of the transcripts were evaluated in relation to sperm characteristics and preimplantation embryo development.The mean(±standard deviation)RNA amount per spermatozoon was 28.48(±23.03)femtogram in the overall group and was significantly higher in the OT group than that in N and T groups.Total sperm RNA and gene expression of PRM1 and PRM2 genes were related to preimplantation embryo development and developmental arrest.Specific sperm characteristics were correlated with the expressions of PRM1,PRM2,or TH2B genes.We conclude that the sperm RNA amount and composition are important factors and might influence early embryonic development and also differ in different cases of male infertility.

    Impact of human papillomavirus and coinfection with other sexually transmitted pathogens on male infertility

    查看更多>>摘要:This study primarily aimed to investigate the prevalence of human papillomavirus(HPV)and other common pathogens of sexually transmitted infections(STIs)in spermatozoa of infertile men and their effects on semen parameters.These pathogens included Ureaplasma urealyticum,Ureaplasma parvum,Chlamydia trachomatis,Mycoplasma genitalium,herpes simplex virus 2,Neisseria gonorrhoeae,Enterococcus faecalis,Streptococcus agalactiae,Pseudomonas aeruginosa,and Staphylococcus aureus.A total of 1951 men of infertile couples were recruited between 23 March 2023,and 17 May 2023,at the Department of Reproductive Medicine of The First People's Hospital of Yunnan Province(Kunming,China).Multiplex polymerase chain reaction and capillary electrophoresis were used for HPV genotyping.Polymerase chain reaction and electrophoresis were also used to detect the presence of other STIs.The overall prevalence of HPV infection was 12.4%.The top five prevalent HPV subtypes were types 56,52,43,16,and 53 among those tested positive for HPV.Other common infections with high prevalence rates were Ureaplasma urealyticum(28.3%),Ureaplasma parvum(20.4%),and Enterococcus faecalis(9.5%).The prevalence rates of HPV coinfection with Ureaplasma urealyticum,Ureaplasma parvum,Chlamydia trachomatis,Mycoplasma genitalium,herpes simplex virus 2,Neisseria gonorrhoeae,Enterococcus faecalis,Streptococcus agalactiae,and Staphylococcus aureus were 24.8%,25.4%,10.6%,6.4%,2.4%,7.9%,5.9%,0.9%,and 1.3%,respectively.The semen volume and total sperm count were greatly decreased by HPV infection alone.Coinfection with HPV and Ureaplasma urealyticum significantly reduced sperm motility and viability.Our study shows that coinfection with STIs is highly prevalent in the semen of infertile men and that coinfection with pathogens can seriously affect semen parameters,emphasizing the necessity of semen screening for STIs.

    Association of redundant foreskin with sexual dysfunction:a cross-sectional study from 5700 participants

    查看更多>>摘要:A previous study showed that the length of the foreskin plays a role in the risk of sexually transmitted infections and chronic prostatitis,which can lead to poor quality of sexual life.Here,the association between foreskin length and sexual dysfunction was evaluated.A total of 5700 participants were recruited from the andrology clinic at The First Affiliated Hospital of University of Science and Technology of China(Hefei,China).Clinical characteristics,including foreskin length,were collected,and sexual function was assessed by the International Index of Erectile Function-5(IIEF-5)and Premature Ejaculation Diagnostic Tool(PEDT)questionnaires.Men with sexual dysfunction were more likely to have redundant foreskin than men without sexual dysfunction.Among the 2721 erectile dysfunction(ED)patients and 1064 premature ejaculation(PE)patients,301(11.1%)ED patients and 135(12.7%)PE patients had redundant foreskin,respectively.Men in the PE group were more likely to have redundant foreskin than men in the non-PE group(P=0.004).Logistic regression analyses revealed that the presence of redundant foreskin was associated with increased odds of moderate/severe ED(adjusted odds ratio[aOR]=1.31,adjusted P=0.04),moderate PE(aOR=1.38,adjusted P=0.02),and probable PE(aOR=1.37,adjusted P=0.03)after adjusting for confounding variables.Our study revealed a positive correlation between the presence of redundant foreskin and the risk of sexual dysfunction,especially in PE patients.Assessment of the length of the foreskin during routine clinical diagnosis may provide information for patients with sexual dysfunction.

    查看更多>>摘要:Peyronie's disease(PD)is a disorder characterized by fibrous plaque formation in the penile tissue that leads to curvature and complications in advanced stages.In this study,we aimed to compare four injectable induction agents for the establishment of a robust rat model of PD:transforming growth factor-β1(TGF-β1),fibrin,sodium tetradecyl sulfate(STS)combined with TGF-β1,and polidocanol(POL)combined with TGF-β1.The results showed that injection of TGF-β1 or fibrin into the tunica albuginea induced pathological endpoints without causing penile curvature.The STS+TGF-β1 combination resulted in both histological and morphological alterations,but with a high incidence of localized necrosis that led to animal death.The POL+TGF-β1 combination produced pathological changes and curvature comparable to STS+TGF-β1 and led to fewer complications.In conclusion,fibrin,STS+TGF-β1,and POL+TGF-β1 all induced PD with a certain degree of penile curvature and histological fibrosis in rats.The POL+TGF-β1 combination offered comparatively greater safety and clinical relevance and may have the greatest potential for PD research using model rats.

    查看更多>>摘要:Chronic prostatitis/chronic pelvic pain syndrome(CP/CPPS)is a complex disease that is often accompanied by mental health disorders.However,the potential mechanisms underlying the heterogeneous clinical presentation of CP/CPPS remain uncertain.This study analyzed widely targeted metabolomic data of expressed prostatic secretions(EPS)and plasma to reveal the underlying pathological mechanisms of CP/CPPS.A total of 24 CP/CPPS patients from The Second Nanning People's Hospital(Nanning,China),and 35 asymptomatic control individuals from First Affiliated Hospital of Guangxi Medical University(Nanning,China)were enrolled.The indicators related to CP/CPPS and psychiatric symptoms were recorded.Differential analysis,coexpression network analysis,and correlation analysis were performed to identify metabolites that were specifically altered in patients and associated with various phenotypes of CP/CPPS.The crucial links between EPS and plasma were further investigated.The metabolomic data of EPS from CP/CPPS patients were significantly different from those from control individuals.Pathway analysis revealed dysregulation of amino acid metabolism,lipid metabolism,and the citrate cycle in EPS.The tryptophan metabolic pathway was found to be the most significantly altered pathway associated with distinct CP/CPPS phenotypes.Moreover,the dysregulation of tryptophan and tyrosine metabolism and elevation of oxidative stress-related metabolites in plasma were found to effectively elucidate the development of depression in CP/CPPS.Overall,metabolomic alterations in the EPS and plasma of patients were primarily associated with oxidative damage,energy metabolism abnormalities,neurological impairment,and immune dysregulation.These alterations may be associated with chronic pain,voiding symptoms,reduced fertility,and depression in CP/CPPS.This study provides a local-global perspective for understanding the pathological mechanisms of CP/CPPS and offers potential diagnostic and therapeutic targets.

    查看更多>>摘要:Primary ciliary dyskinesia(PCD)is a clinically rare,genetically and phenotypically heterogeneous condition characterized by chronic respiratory tract infections,male infertility,tympanitis,and laterality abnormalities.PCD is typically resulted from variants in genes encoding assembly or structural proteins that are indispensable for the movement of motile cilia.Here,we identified a novel nonsense mutation,c.466G>T,in cilia-and flagella-associated protein 300(CFAP300)resulting in a stop codon(p.Glu156*)through whole-exome sequencing(WES).The proband had a PCD phenotype with laterality defects and immotile sperm flagella displaying a combined loss of the inner dynein arm(IDA)and outer dynein arm(ODA).Bioinformatic programs predicted that the mutation is deleterious.Successful pregnancy was achieved through intracytoplasmic sperm injection(ICSI).Our results expand the spectrum of CFAP300 variants in PCD and provide reproductive guidance for infertile couples suffering from PCD caused by them.

    Application of genome tagging technology in elucidating the function of sperm-specific protein 411(Ssp411)

    查看更多>>摘要:The genome tagging project(GTP)plays a pivotal role in addressing a critical gap in the understanding of protein functions.Within this framework,we successfully generated a human influenza hemagglutinin-tagged sperm-specific protein 411(HA-tagged Ssp411)mouse model.This model is instrumental in probing the expression and function of Ssp411.Our research revealed that Ssp411 is expressed in the round spermatids,elongating spermatids,elongated spermatids,and epididymal spermatozoa.The comprehensive examination of the distribution of Ssp411 in these germ cells offers new perspectives on its involvement in spermiogenesis.Nevertheless,rigorous further inquiry is imperative to elucidate the precise mechanistic underpinnings of these functions.Ssp411 is not detectable in metaphase Ⅱ(MⅡ)oocytes,zygotes,or 2-cell stage embryos,highlighting its intricate role in early embryonic development.These findings not only advance our understanding of the role of Ssp411 in reproductive physiology but also significantly contribute to the overarching goals of the GTP,fostering groundbreaking advancements in the fields of spermiogenesis and reproductive biology.

    查看更多>>摘要:Parameters of peripheral blood cell have been shown as the potential predictors of erectile dysfunction(ED).To investigate the clinical significance of hematological parameters for predicting the risk of rapid ejaculation,we established a rat copulatory model on the basis of ejaculation distribution theory.Blood samples from different ejaculatory groups were collected for peripheral blood cell counts and serum serotonin(5-HT)tests.Meanwhile,the relationship between hematological parameters and ejaculatory behaviors was assessed.Final analysis included 11 rapid ejaculators,10 normal ejaculators,and 10 sluggish ejaculators whose complete data were available.The platelet(PLT)count in rapid ejaculators was significantly lower than that in normal and sluggish ejaculators,whereas the platelet distribution width(PDW)and mean platelet volume(MPV)were significantly greater in rapid ejaculators.Multivariate logistic regression analysis and receiver operating characteristic(ROC)curve analysis showed that the PLT was an independent protective factor for rapid ejaculation.Meanwhile,rapid ejaculators were found to have the lowest serum 5-HT compared to normal and sluggish ejaculators(P<0.001).Furthermore,there was a positive correlation between the PLT and serum 5-HT(r=0.662,P<0.001),indicating that the PLT could indirectly reflect the serum 5-HT concentration.In addition,we assessed the association between the PLT and ejaculatory parameters.There was a negative correlation between ejaculation frequency(EF)and the PLT(r=-0.595,P<0.001),whereas there was a positive correlation between ejaculation latency(EL)and the PLT(r=0.740,P<0.001).This study indicated that the PLT might be a useful and convenient diagnostic marker for predicting the risk of rapid ejaculation.
