    STING1 in sepsis:Mechanisms,functions,and implications

    查看更多>>摘要:Sepsis is a life-threatening clinical syndrome and one of the most challenging health problems in the world.Pathologically,sepsis and septic shock are caused by a dysregulated host immune response to infection,which can eventually lead to multiple organ failure and even death.As an adaptor transporter between the endoplasmic reticulum and Golgi apparatus,stimulator of interferon response cGAMP interactor 1 (STING1,also known as STING or TMEM173) has been found to play a vital role at the intersection of innate immunity,inflammation,autophagy,and cell death in response to invading mi-crobial pathogens or endogenous host damage.There is ample evidence that impaired STING1,through its immune and non-immune functions,is involved in the pathological process of sepsis.In this review,we discuss the regulation and function of the STING1 pathway in sepsis and highlight it as a suitable drug target for the treatment of lethal infection.

    查看更多>>摘要:Chronic wounds have always been a tough fight in clinical practice,which can not only make patients suffer from pain physically and mentally but also impose a heavy burden on the society.More than one factor is relevant to each step of the development of chronic wounds.Along with the in-depth research,we have realized that figuring out the pathophysiological mechanism of chronic wounds is the foun-dation of treatment,while wound infection is the key point concerned.The cause of infection should be identified and prevented promptly once diagnosed.This paper mainly describes the mechanism,diag-nosis and therapeutic strategies of chronic wound infection,and will put an emphasis on the principle of debridement.

    查看更多>>摘要:Purpose:COVID-19 is also referred to as a typical viral septic pulmonary infection by 2019-nCoV.However,little is known regarding its characteristics in terms of systemic inflammation and organ injury,especially compared with classical bacterial sepsis.This article aims to investigate the clinical charac-teristics and prognosis between COVID-19-associated sepsis and classic bacterial-induced sepsis.Methods:In this retrospective cohort study,septic patients with COVID-19 in the intensive care unit(ICU) of a government-designed therapy center in Shenzhen,China between January 14,2020 and March 10,2020,and septic patients induced by carbapenem-resistant klebsiella pneumonia (CrKP) admitted to the ICU of the Second People's Hospital of Shenzhen,China between January 1,2014 and October 30,2019 were enrolled.Demographic and clinical parameters including comorbidities,critical illness scores,treatment,and laboratory data,as well as prognosis were compared between the two groups.Risk factors for mortality and survival rate were analyzed using multivariable logistic regression and survival curve,respectively.Results:A total of 107 patients with COVID-19 and 63 patients with CrKP were enrolled.A direct com-parison between the two groups demonstrated more serious degrees of primary lung injury following 2019-nCoV infection (indicated by lower PaO2/FiO2),but milder systemic inflammatory response,lower sequential organ failure assessment score and better functions of the organs like heart,liver,kidney,coagulation,and circulation.However,the acquired immunosuppression presented in COVID-19 patients was more severe,which presented as lower lymphocyte counts (0.8× 109/L vs.0.9× 109/L).Moreover,the proportion of COVID-19 patients treated with corticosteroid therapy and extracorporeal membrane oxygenation was larger compared with CrKP patients (78.5% vs.38.1% and 6.5% vs.0,respectively) who required less invasive mechanical ventilation (31.6% vs.54.0%).The incidence of hospitalized mortality and length of ICU stay and total hospital stay were also lower or shorter in viral sepsis (12.1% vs.39.7%,6.5 days vs.23.0 days and 21.0 days vs.33.0 days,respectively) (all p < 0.001).Similar results were obtained after being adjusted by age,gender,comorbidity and PaO2/FiO2.Lymphocytopenia and high acute physiology and chronic health evaluation Ⅱ scores were common risk factors for in-hospital death.While the death cases of COVID-19 sepsis mostly occurred at the later stages of patients' hospital stay.Conclusion:Critical COVID-19 shares clinical characteristics with classical bacterial sepsis,but the degree of systemic inflammatory response,secondary organ damage and mortality rate are less severe.However,following 2019-nCoV infection,the level of immunosuppression may be increased and thus induce in more death at the later stage of patients' hospitalstay.

    查看更多>>摘要:Purpose:To investigate the clinical value of urine interleukin-18 (IL-8),neutrophil gelatinase-associated lipocalin (NGAL) and kidney injury molecule-1 (KIM-1) for the early diagnosis of acute kidney injury(AKI) in patients with ureteroscopic lithotripsy (UJRL) related urosepsis.Methods:A retrospective study was carried out in 157 patients with urosepsis after URL.The patients were divided into AKI group and non-AKI group according to the Kidigo guideline and urine IL-8,NGAL and KIM-1 levels were detected by enzyme-linked immunosorbent assay at 0,4,12,24 and 48 h after the surgery.Receiver operating characteristic curve (ROC) was used to evaluate the diagnostic value of these three biomarkers for postoperative AKI.Results:The level of urine IL-8,NGAL and KIM-1 in AKI group was significantly higher than that in non-AKI group at 4,12,24 and 48 h (p < 0.01).The ROC analysis showed the combined detection of urine IL-8,NGAL and KIM-1 at 12 h had a larger area under curve (AUC) than a single marker (0.997,95% CI:0.991-0.998),and the sensitivity and specificity were 98.2% and 96.7%,respectively.Pearson correlation analysis showed that the levels of urine NGAL at 4,12,24 and 48 h in AKI patients were positively correlated with the levels of urine KIM-1 and IL-18 (p < 0.01).Conclusion:AKI could be quickly recognized by the elevated level of urine IL-8,NGAL and KIM-1 in patients with URL-related urosepsis.Combined detection of the three urine biomarkers at 12 h after surgery had a better diagnostic performance,which may be an important reference for the early diag-nosis of AKI.

    Mixed bacterial-fungal infection following total hip arthroplasty:A case report

    查看更多>>摘要:Prosthetic infection is one of the severe postoperative complications of arthroplasty.Mixed bacterial-fungal prosthetic infection is rare but can be disastrous.This case was a 76-year-old female suffered from prosthetic infection following total hip replacement due to femoral neck fracture and underwent multiple debridements.The culture of periprosthetic tissue was bacteriologically sterile following the first debridement,while the Staphylococcus hominis was identified in the second debridement in the previous hospitalization where fungal infection had not been considered.Thus the pathogen spectrum of anti-infection therapy failed to contain fungus.Ultimately,the culture result of our sampled peri-prosthetic tissue during the third debridement was Candida albicans without bacterium in our hospital.The fungal prosthetic infection was successfully treated by a two-stage revision with antifungal drugs.Accurate diagnosis and standardized treatment is the key to the therapy of infection after hip arthro-plasty,especially for mixed bacterial-fungal prosthetic infection.

    查看更多>>摘要:Purpose:To retrospectively analyze the clinical outcomes of meniscus repair with simultaneous anterior cruciate ligament (ACL) reconstruction and explore the causes of failure of meniscus repair.Methods:From May 2013 to July 2018,the clinical data of 165 patients who were treated with meniscus surgery and simultaneous ACL reconstruction,including 69 cases of meniscus repair (repair group) and 96 cases of partial meniscectomy (partial meniscectomy group) were retrospectively analyzed.The exclusion criteria were as follows:(1) ACL rupture associated with fracture,collateral ligament injury,or complex ligament injury;(2) a history of knee surgery;or (3) a significant degree of osteoarthritis.The 69 patients in the repair group were divided into the non-failure group (62 cases) and the failure group (7 cases) depending on the repair effect.Postoperative outcomes of the repair group and the partial meniscectomy group were compared,General conditions and postoperative outcomes of the failure group and the non-failure group were compared.During the median follow-up period of 28 months(range,4-65 months) after the second arthroscopy,postoperative outcomes of seven patients in the failure group were summarized.SPSS 25.0 statistical software was used for statistical analysis.A p value less than 0.05 was considered statistically significant.Results:Seven patients in the failure group who underwent the second arthroscopy were followed up for(30 _± 17.4) months and their postoperative outcomes were summarized.Compared with the partial meniscectomy group,the International Knee Documentation Committee scores of patients in the repair group improved significantly (p =0.031).Compared with the non-failure group,more patients in the failure group were younger than 24 years (p =0.030).The median follow-up period was 39.5 months.All patients recovered well after subsequent partial meniscectomy and relieved clinical symptoms.Visual analog scale scores decreased significantly (p =0,026),and the International Knee Documentation Committee and Lysholm scores improved significantly (p =0.046 for both).Conclusion:The failure rate of meniscus repair in this study was 10.1% (7/69),all of which were medial meniscus tears.However,the surgical outcomes of ACL reconstruction were not affected,and there might be a role for graft protection.Therefore,meniscus retears can be successful treated by performing subsequent partial meniscectomy in patients with repair failure.

    查看更多>>摘要:Purpose:Rib fractures are one of the most common causes of morbidity and mortality and are associated with abdominal solid organ injury (ASOI).The purpose of this study was to investigate the correlation of ASOI with the number,location,and involved segments of rib fracture(s) in blunt chest trauma.Methods:This retrospective cohort study was conducted on patients with blunt chest trauma over the age of 15 years,who were hospitalized with the diagnosis of rib fractures from July 2015 to September 2020.After ethic committee approval,a retrospective chart review was designed and patients with a diagnosis of rib fractures were selected.Patients who had chest and abdominopelvic CT scan were included in the study and additional data including age,gender,injury severity score,trauma mecha-nism,number and sides of the fractured ribs (left/right/bilateral),rib fracture segments (upper,middle,lower zone) and results of chest and abdominal spiral CT scan were recorded.The correlation between ASOI and the sides,segments and number of rib fracture(s) was assessed by Pearson's correlation coefficient.Results:Altogether 1056 patients with rib fracture(s) were included.The mean age was (42.76 ± 13.35)years and 85.4% were male.The most common mechanism of trauma was car accident (34.6%).Most fractures occurred in the middle rib zone (60.44%) and the most commonly involved ribs were the 6th and 7th ones (15.7% and 16.4%,respectively).Concurrent abdominal injuries were observed in 103 pa-tients (34.91%) and were significantly associated with middle zone rib fractures.Conclusion:There is a significant relationship between middle zone rib fractures and ASOI.Intra-abdominal injuries are not restricted to fractures of the lower ribs and thus should always be kept in mind during management of blunt trauma patients with rib fractures.

    Mini suture anchor:An effective device for reduction and fixation of displaced temporomandibular joint disc with intracapsular condylar fracture

    查看更多>>摘要:Purpose:The main aim is to provide clinical reference for the application of mini suture anchor in the reduction and fixation of displaced temporomandibular joint (TMJ) disc with intracapsular condylar fracture.Methods:From October 2018 to October 2019,21 patients (31 sides) with intracapsular condylar frac-tures and articular disc displacement from West China Hospital of Stomatology,Sichuan University were included.The selection criteria were:(1) mandibular condylar fractures accompanied by displacement of the TMJ disc,confirmed by clinical examination,CT scan and other auxiliary examinations;(2) indication for surgical treatment;(3) no surgical contraindications;(4) no previous history of surgery in the operative area;(5) no facial nerve injury before the surgery;(6) informed consent to participate in the research program and (7) complete data.Patients without surgical treatment were excluded.The employed patients were followed up at 1,3,6 and 12 months after operation.Outcomes were assessed by success rate of operation,TMJ function and radiological examination results at 3 months after operation.Data were expressed as number and percent and analyzed using SPSS 19.0.Results:All the surgical procedures were completed successfully and all the articular discs were firmly attached to the condyles.The articular disc sufficiently covered the condylar head after the fixation.The fixation remained stable when the mandible was moved in each direction by the surgeons,No com-plications occurred.The functions of the TMJ were well-recovered postoperatively in most cases.CT scan revealed that the screws were completely embedded in the bone without loosening or displacement.Conclusion:Mini suture anchor can provide satisfactory stabilization for the reduced articular disc and also promote the recovery of TMJ functions.

    查看更多>>摘要:Wide-awake local anesthesia has many advantages.We describe a new method to use wide-awake local anesthesia with more flexibility.A 32-year-old man with a severe right-hand contracture after an iat-rogenic tourniquet accident during an anterolateral thigh flap for a partial hand amputation underwent contracture release using external fixation after proximal row carpectomy and subsequent tenolysis.We performed most of the tenolysis procedure under general anesthesia and the final stage with an intra-operative assessment of active finger movement and dissection under local anesthesia.He regained his grip strength 2.5 years post-injury.General anesthesia is useful to treat a surgical site with extensive hard scars,whereas local anesthesia is useful for adjusting tension in an awake patient.The indication for wide-awake surgery is yet to be established;our method of combining general and local anesthesia in the tenolysis procedure illustrates the possibilities in expanding this method.
