    Military training-related abdominal injuries and diseases:Common types,prevention and treatment

    查看更多>>摘要:Military training is intense,difficult and often dangerous,so all kinds of injuries or diseases frequently occur during training.Most of the previous studies and reviews on military training-related injuries focused on musculoskeletal system,whereas there are no reviews of abdominal injuries and diseases.Although the incidence of military training-related abdominal injuries and diseases is relatively low,the patients'condition is often critical especially in the presence of abdominal organ injury,leading to multi-organ dysfunction syndrome and even death.This paper elaborates on common types of military training-related abdominal injuries and diseases as well as the prevention and treatment measures,which provides some basis for scientific and reasonable training and improvement of medical security.

    Establishment of a combat damage control surgery training platform for explosive combined thoraco-abdominal injuries

    查看更多>>摘要:Purpose:It is challenging to prepare military surgeons with the skills of combat damage control surgery(CDCS).The current study aimed to establish a damage control surgery(DCS)training platform for explosive combined thoraco-abdominal injuries.Methods:The training platform established in this study consisted of 3 main components:(1)A 50 m×50 m square yard was constructed as the explosion site.Safety was assessed through cameras.(2)Sixteen pigs were injured by an explosion of trinitrotoluene attached with steel balls and were randomly divided into the DCS group(accepted DCS)and the control group(have not accepted DCS).The mortality rate was observed.(3)The literature was reviewed to identify the key factors for assessing CDCS,and testing standards for CDCS were then established.Expert questionnaires were employed to evaluate the scientificity and feasibility of the testing standards.Then,a 5-day training course with incorporated tests was used to test the efficacy of the established platform.In total,30 teams attended the first training course.The scores that the trainees received before and after the training were compared.SPSS 11.0 was employed to analyze the results.Results:The high-speed video playback confirmed the safety of the explosion site as no explosion fragments projected beyond the wall.No pig died within 24 h when DCS was performed,while 7 pigs died in the control group.After a literature review,assessment criteria for CDCS were established that had a total score of 100 points and had 4 major parts:leadership and team cooperation,resuscitation,surgical procedure,and final outcome.Expert questionnaire results showed that the scientific score was 8.6±1.25,and the feasibility score was 8.74±1.19.When compared with the basic level,the trainees'score improved significantly after training.Conclusion:The platform established in this study was useful for CDCS training.

    查看更多>>摘要:Purpose:The data concerning long-term follow-up and outcomes of penetrating trauma are poorly detailed in the literature.The main objective of our study was to analyze the hospital and extra-hospital follow-up of penetrating trauma victims and to evaluate the late complications and long-term conse-quences of these traumas.Methods:This work was a retrospective longitudinal monocentric observational study conducted at Laveran Military Hospital,from January 2007 to January 2017.All patients hospitalized for gunshot wound or stab wound management during this period were identified via a retrospective systematic query in the hospital information system using the ICD-10 codes.Epidemiological data,traumatism characteristics,hospital management,follow-up and traumatism consequences(i.e.,persistent disability)were analyzed.To improve evaluation of traumatism long-term consequences,extra-hospital follow-up data from general physicians(GP)were collected by phone call.During this interview,9 closed questions were asked to the GP.The survey evaluated:the date of the last consultation related to injury with the GP,the specific follow-up carried out by the GP,traumatism consequences,and recurrence of traumatism.Descriptive,univariate and multivariate with regression analysis were used for statistical analysis.Results:A total number of 165 patients were included.Median(Q1,Q3)of hospital follow-up was 28(4,66)days.One hundred one patients(61.2%)went to their one-month consultation at hospital.GP follow-up was achieved for 76 patients(55.2%).Median(Q1,Q3)of GP follow-up was 47(21,75)months.Twenty-four patients(14.5%)have been totally lost to follow up.The overall follow-up identified 54 patients(32.7%)with long-term consequences,20 being psychiatric disorders and 30 organic injuries.Organic consequences were mainly peripheral nerve damages(n=20;12.1%).Most of the psychiatric consequences were diagnosed during GP follow-up(n=14;70%).Seventeen cases(10.3%)of recurrence were found and late mortality occurred in 4 patients(2.4%).High injury severity score,older age and gunshot wound were significantly linked to long-term consequences.Data collection and analysis were carried out in accordance with MR004 reference methodology.Conclusion:This study showed a high rate of long-term consequences among patients managed for penetrating injury.If all organic lesions are diagnosed during hospital follow-up and jointly managed by hospital and extra-hospital physicians,most socio-psychiatric consequences were detected and followed by extra-hospital workers.However,for half of the patients,the extra-hospital follow-up could not be assessed.Thus,these consequences are very probably underestimated.It appears imperative to strengthen the compliance and adherence of these patients to the care network.Awareness and involvement of medical,paramedical teams and GP role seems essential to screen and manage these consequences.

    Influence of impact velocity and impact attack angle of bullets on damage of human tissue surrogate——ballistic gelatin

    查看更多>>摘要:Purpose:Terminal performance of a bullet in human body is critical for the treatment of gunshot injury and optimization of bullet design.The effects of the impact velocity(v0)and the impact attack angle(δ0)of the bullet on its terminal performance was investigated,using a new evaluation method(called expansion method)based on the expansion of cracks and the permanent cavity wall in ballistic gelatin.Methods:Ballistic gelatin was used to simulate human body.The 7.62 mm x 39 mm rifle bullets with different v0(600-760 m/s)and δ0(0°-6°)were fired into the gelatin blocks.The gelatin block was cut into slices of about 20 mm thickness.The cracks and the permanent cavity on each slice were obtained manually.The damaged gelatin was determined using two methods:expanding the permanent cavity but ignoring the cracks,and expanding both the permanent cavity and the cracks.The relations between the damaged gelatin and v0 and δ0 were obtained using linear fitting method.Results:According to the distribution of the damaged gelatin along the penetration depth,the damaged gelatin block could be divided into two parts:the less damaged part and the severely damaged part.The length of the less damaged part depends mostly on δ0;while the average damaged area of this part depends on both δ0 as well as v0.The cracks contributed significantly to the total volume of damaged gelatin,particularly when the expansion was larger than 1.9 mm.The total damaged gelatin increases with v0,δ0 and the expansion extent.The average length of equivalent cracks grew with v0 and δ0 when considering the cracks,and decreased with v0 when ignoring the cracks.Conclusion:The expansion method is suitable to investigate the influence of different factors of bullets on their terminal performance.The characteristics of the damaged gelatin have a linear relationship with the initial attack angle(δ0)and the initial velocity(v0)of the bullet.

    The role of altered fatty acid in pathological scars and their dermal fibroblasts

    查看更多>>摘要:Purpose:The proposed pathological mechanism for scar formation is controversial,and increased attention has been paid to the fatty acids(FAs)in the formation of pathological scars.Notably,FAs are known to be important in inflammation and mechanotransduction,which is closely related to scar formation.Therefore,it is necessary to clarify the roles of FA in scar formation.Methods:Hypertrophic scar and keloid formed for more than a year and without other treatment,as well as normal skin samples were obtained from patients who underwent plastic surgery.Finally,keloids(n=10),hypertrophic scars(n=10),and normal skin samples(n=10)were collected under informed consent.Primary dermal fibroblasts were isolated and cultured.The amount and variety of FAs were detected by lipid chromatography-mass spectrometry.Immunohistochemistry,real-time PCR,and western blotting were used to verify the expression of sterol regulatory element-binding protein-1(SREBP1)and fatty acid synthase(FASN)in the samples and their fibroblasts.Student's t-test,ANOVA,and orthogonal partial least square discriminant analysis were performed for statistical analysis(*p<0.05,**p<0.01,***p<0.001,****p<0.0001).Results:Compared with full-thickness normal skin,there were 27 differential FAs in keloids and 15 differential FAs in hypertrophic scars(*p<0.05 and variable influence on projection>1.0).The expression of SREBP1 and FASN was lower in pathological scars both at mRNA and protein levels(all*p<0.05).However,the mRNA levels of SREBP1(***p=0.0002)and FASN(***p=0.0021)in keloid-derived fibroblasts were higher than that in normal skin fibroblasts(NFBs),while the expression in hypertrophic scar-derived fibroblasts was lower than that in NFBs(both *p<0.05).Whereas there was no significant difference in FASN protein expression between keloid-derived fibroblasts and NFBs(p>0.05).Conclusion:FAs involved in pathological scars are abnormally changed in scar formation.Thus,fatty acid-derived inflammation and de novo synthesis pathway of FA may play a key role in the formation of pathological scars.

    查看更多>>摘要:Purpose:Common peroneal nerve palsy is quite disabling and every effort should be made to prevent its injury during the treatment.Methods:We retrospectively reviewed the prospectively collected data of 7 cases of tibial plateau fractures in association with proximal fibula fracture from January 2019 to September 2019 who pre-sented to emergency room of our hospital.Results:In addition to fibular neck fracture,the first case had type 6 tibial plateau displaced fracture and the second case had displaced acetabular fracture with instability of knee with tibial tuberosity avulsion.common peroneal nerve palsy developed following application of distal tibial skeletal traction in both the cases.Other 6 such cases remained neurologically intact as traction was not applied to them.Conclusion:Such iatrogenic complication could have been prevented if the injury pattern of"concomi-tant medial and lateral columns"of the proximal leg is kept in mind by the treating surgeon before applying skeletal traction.

    查看更多>>摘要:Purpose:No therapeutic consensus has been established about proximal ruptures of the rectus femoris muscle.The objective of this literature review is to determine a therapeutic course of action.Methods:We conducted a literature review on the PubMed database using the following keywords(in French and English,respectively):"quadriceps/quadriceps","droit antérieur/rectus femoris","proximal/proximal","chirurgie/surgical","avulsion/avulsion".We collected 266 articles,36 of them were selected,which were related to our topic:proximal rupture of the anterior rectus femoris.Patients with a proximal rupture of the rectus femoris,minor or major patient of traumatic origin were included in this study.Patients injured at another lesion level,or non-traumatic lesions of the proximal rectus femoris(tendinitis without ruptures,tumor or others)were excluded.For each patient,the indications,the type of treatment and the functional result were analyzed,with the time to recovery and the level of recovery from sports and professional activities(same sport/profession or not,same level or not)as the main criterion of judgment.Fisher exact test was used for statistical comparison.Results:The aims of conservative treatment are to be pain free for the patient,to fight hematoma and to rehabilitate the injury as quickly as possible.The surgical techniques are varied,with most consisting of either a reinsertion of the musculo-tendon stump or a resection of the scar tissue with myo-tendino-aponeurotic suture in place.The functional results are good for the majority of the treatments pro-posed,but the conservative treatment has a shorter recovery time(3 months vs.4 months for the best surgical results).Highly displaced bone avulsion is the only indication for first-line surgical treatment.Conslusion:The main disadvantage of conservative treatment is the risk of residual pain beyond 3 months(10%),justifying an MRI to guide secondary surgical treatment.We propose a treatment plan for proximal rupture of the proximal rectus femoris rupture.

    查看更多>>摘要:Anterior tibiofemoral dislocation after total knee arthroplasty is an extremely rare and serious event.Amongst English-published papers,we found only 15 relevant cases,3 of which presented vascular complications.This manuscript aims to present a 77-year-old woman with a TC-Plus(Smith&Nephew)cruciate-retaining type in first time of knee prosthesis,who suffered an anterior tibiofemoral dislocation and were admitted to our hospital.The clinical management and outcome were evaluated.Furthermore,a review of literature was performed.We concluded that early detection and surgical intervention of vascular injury is the key in the survival of the limbs.If there is still knee instability after acute recovery,it seems that revision surgery with constrained total knee arthroplasty can bring about good clinical and functional results.

    查看更多>>摘要:Common or external iliac artery injury due to blunt trauma is unusual without an associated pelvic fracture.Here we report on a 62 years old man that sustained left external iliac artery thrombosis due to blunt trauma following fall from motorbike.There was no immediate circulatory compromise.Contrasted CT of abdomen revealed an associated left lower abdominal wall traumatic hernia.The iliac artery was intervened with an endovascular stent to restore luminal flow and the hernia was repaired electively.The entire clinical course and management dilemma are described in this article.

    查看更多>>摘要:Bipolar clavicular dislocation is rare,and therefore its management is contentious.With an increase of patient's physical demand and a near-normal shoulder function,there has been a shift in the paradigm of its management from a conservative one to a stabilized one of anatomical open reduction.Proposed methods of fixation have also evolved with the advent of more biological fixation devices,which elude implant or fixation related complications.This case report highlights the salient features of this rare case and details the management options along with the benefits of biological anatomical repair and reconstruction.
