    Impact of dental and orofacial trauma on oral health-related quality of life in adults:A systematic review

    查看更多>>摘要:Purpose:Dental and orofacial trauma among the adult population constitutes a major public health problem.The impact is not just physical but also psychological.To analyse the impacts of dental and orofacial trauma on oral health-related quality of life(OHRQoL)in adults and determine whether the 2 variables are closely interlinked.Methods:This is a systematic review.The terms"dental trauma","orofacial trauma","oral health","oral health related impact life","OHRQoL","positive and negative affect scale","quality of life","facial in-juries","adults",and"young adults"were researched in the databases of PubMed,ScienceDirect,Scopus and Google Scholar for associated studies up to December 30,2022.A comprehensive search was designed and the articles were independently screened for eligibility by 2 reviewers.The included studies'author,year of publication,the country where the study was conducted,population de-mographics(number and age),an instrument used for assessing OHRQoL and the relevant result were recorded and compared.The quality of the evidence was assessed using Joanna Briggs Institute checklist for observational studies.Results:Out of 482 unique records,3 articles were included for data extraction.Observational studies were included.Two studies did not mention confounding factors.Different scales were used for dental and orofacial trauma and OHRQoL.OHRQoL has a directly proportional relationship with orofacial trauma.Adolescents with orofacial trauma have a significant impact on this value with a prevalence of 88.4%.Conclusion:The highest impact on OHRQoL was seen immediately after the diagnosis of an orofacial trauma.The impact increases with the severity of the trauma.Therefore,to promote overall dental and general health,health education initiatives should include information on the causes,prevention,and requirement for prompt responses by the populace in seeking dental intervention.

    An experimental study of a novel external fixator based on universal joints for the rapid initial stabilization of mandibular comminuted fractures

    查看更多>>摘要:Purpose:Shaping and assembling contemporary external fixators rapidly for the severe mandibular fractures remains a challenge,especially in emergency circumstance.We designed a novel external fixator that incorporates universal joints to provide the stabilization for mandibular comminuted frac-tures.This study aims to confirm the efficacy of this novel external fixator through biomechanical tests in vitro and animal experiments.Methods:In vitro biomechanical tests were conducted using 6 fresh canine with mandibular defect to simulate critical comminuted fractures.Three mandibles were stabilized by the novel external fixator and other mandibles were fixed by 2.5 mm reconstruction plates.All fixed mandibles were subjected to loads of 350 N on the anterior regions of teeth and 550 N on the first molar of the unaffected side.The stability was evaluated based on the maximum displacement and the slope of the load-displacement curve.In animal experiments,9 beagles with comminuted mandibular fractures were divided into 3 groups,which were treated with the novel external fixation,reconstruction plate,and dental arch bar,respectively.The general observation,the changes in animals'weight,and the surgical duration were recorded and compared among 3 groups.The CT scans were performed at various intervals of 0 day(immediately after the surgery),3 days,7 days,14 days,21 days,and 28 days to analyze the displacement of feature points on the canine mandible and situation of fracture healing at 28 days.The statistical significance was assessed by the two-way analysis of variance test followed by the Bonferroni test,enabling multiple comparisons for all tests using GraphPad Prism10.1.0(GraphPad Inc,USA).Results:The outcomes of the biomechanical tests indicated that no statistically significant differences were found in terms of the maximum displacement(p=0.496,0.079)and the slope of load displacement curves(p=0.374,0.349)under 2 load modes between the external and internal fixation groups.The animal experiment data showed that there were minor displacements of feature points between the external and internal fixation groups without statistic difference,while the arch bar group demonstrated inferior stability.The CT analysis revealed that the best fracture healing happened in the internal fixation group,followed by the external fixation and arch baring at 28 days after fixation.The external fixation group had the shortest fixation duration(25.67±3.79)min compared to internal fixation((70.67±4.51)min,p<0.001)and arch baring((42.00±3.00)min,p=0.046).Conclusion:The conclusion of this study highlighted the efficacy and reliability of this novel external fixator in managing mandibular fractures rapidly,offering a viable option for the initial stabilization of comminuted mandibular fractures in the setting of emergency rescue.

    查看更多>>摘要:Purpose:Trauma accounts for the leading cause of morbidity and mortality worldwide in the present day and may rightly be called the new pandemic.The prominent nature of the face exposes it to various traumatic injuries.A timely,prompt diagnosis along with employment of correct and quick treatment greatly improves the outcome for these patients.The aim of this retrospective study was to analyse the characteristics of maxillofacial injuries over a decade.Methods:The data were collected manually from the medical records of patients who reported to the tertiary centre from 1 January 2011 to 31 December 2019.All injured patients irrespective of age/gender with complete hospital records of clinical and radiographical diagnosis of maxillofacial injuries were included.The demographic data,etiology,site and type of injury,and seasonal variation were analyzed.Data were tabulated into 6 age groups(0-7 years,8-18 years,19-35 years,36-40 years,41-59 years,and>60 years).Five etiological factors,i.e.road traffic accidents,falls,assaults,sports-related,and occupational accidents,were further evaluated based on genders.Facial injuries were classified into 6 types:panfacial fractures,mandibular fractures(subcategorized),midface fractures(subcategorized),dentoalveolar fractures,dental injuries,and soft tissue injuries.The monthly and seasonal variation of the injuries was also charted.Data were expressed as frequency and percent.Results:A total of 10,703 maxillofacial injuries were included from the tertiary centre from the period of 2011-2019,including 8637 males and 2066 females,with the highest occurrence in the 19-35 years age group.Road traffic accident was the principal etiological factor of maxillofacial injuries in both genders(80.5%),followed by falls(9.6%),assaults(8.0%),occupational accidents(1.2%),and sporting injuries(0.7%).Midface fractures accounted for 52.5%(5623 fractures),followed by mandibular fractures(38.1%).Conclusion:The current study describes a change in the incidence of maxillofacial injuries along with variation in the demographic data.The implementation of safety gears and stricter traffic laws along with public awareness may aid in the reduction of maxillofacial injuries.

    A retrospective study of occlusal reconstruction in patients with old jaw fractures and dentition defects

    查看更多>>摘要:Purpose:This study evaluated the methods and clinical effects of multidisciplinary collaborative treat-ment for occlusal reconstruction in patients with old jaw fractures and dentition defects.Methods:Patients with old jaw fractures and dentition defects who underwent occlusal reconstruction at the Third Affiliated Hospital of Air Force Military Medical University from January 2018 to December 2022 were enrolled.Clinical treatment was classified into 3 phases.In phase I,techniques such as orthognathic surgery,microsurgery,and distraction osteogenesis were employed to reconstruct the correct 3-dimensional(3D)jaw position relationship.In phase Ⅱ,bone augmentation and soft tissue management techniques were utilized to address insufficient alveolar bone mass and poor gingival soft tissue conditions.In phase Ⅲ,implant-supported overdentures or fixed dentures were used for occlusal reconstruction.A summary of treatment methods,clinical efficacy evaluation,comparative analysis of imageological examinations,and satisfaction questionnaire survey were utilized to evaluate the thera-peutic efficacy in patients with traumatic old jaw fractures and dentition defects.All data are summa-rized using the arithmetic mean±standard deviation and compared using independent sample t-tests.Results:In 15 patients with old jaw fractures and dentition defects(an average age of 32 years,ranging from 18 to 53 years),there were 7 cases of malocclusion of single maxillary fracture,6 of malocclusion of single mandible fracture,and 2 of malocclusion of both maxillary and mandible fractures.There were 5 patients with single maxillary dentition defects,2 with single mandibular dentition defects,and 8 with both maxillary and mandibular dentition defects.To reconstruct the correct 3D jaw positional rela-tionship,5 patients underwent Le Fort I osteotomy of the maxilla,3 underwent bilateral sagittal split ramus osteotomy of the mandible,4 underwent open reduction and internal fixation for old jaw frac-tures,3 underwent temporomandibular joint surgery,and 4 underwent distraction osteogenesis.All patients underwent jawbone augmentation,of whom 4 patients underwent a free composite vascular-ized bone flap(26.66%)and the remaining patients underwent local alveolar bone augmentation.Free gingival graft and connective tissue graft were the main methods for soft tissue augmentation(73.33%).The 15 patients received 81 implants,of whom 11 patients received implant-supported fixed dentures and 4 received implant-supported removable dentures.The survival rate of all implants was 93.82%.The final imageological examination of 15 patients confirmed that the malocclusion was corrected,and the clinical treatment ultimately achieved occlusal function reconstruction.The patient satisfaction ques-tionnaire survey showed that they were satisfied with the efficacy,phonetics,aesthetics,and comfort after treatment.Conclusion:Occlusal reconstruction of old jaw fractures and dentition defects requires a phased sequential comprehensive treatment,consisting of 3D spatial jaw correction,alveolar bone augmentation and soft tissue augmentation,and implant-supported occlusal reconstruction,achieving satisfactory clinical therapeutic efficacy.

    A serial case study of the combined use of intraoperative CT and surgical navigation system for the removal of small foreign bodies in the maxillofacial region

    查看更多>>摘要:Purpose:The removal of small foreign bodies embedded within the deep soft tissues of the maxillofacial region is a complex and challenging task for maxillofacial surgeons.The purpose of this study was to explore the efficacy of the combination of intraoperative CT and surgical navigation for the removal of small foreign objects in the maxillofacial region.Methods:A serial case study was conducted involving all consecutive patients who underwent surgical removal of small foreign bodies in the maxillofacial region.The combination of intraoperative CT and a surgical navigation system was used at a single medical institution from January 2018 to December 2022.Comprehensive data,including patient demographics,characteristics of the foreign bodies,previous surgical interventions,duration of the surgical procedure,and removal success rate were collected for this study.Relevant data were recorded into Microsoft Excel sheet and analyzed using SPSS version 22.0.Results:Nine patients(6 males and 3 females)were included in this study,with an average age of 37 years.Each patient had previously undergone an unsuccessful removal attempt utilizing conventional surgical methods based on preoperative CT imaging or C-arm guidance at a local healthcare facility.Four patients also experienced unsuccessful attempts with preoperative CT image-based navigation systems.However,by employing the combined approach of intraoperative CT and surgical navigation,the foreign bodies were successfully removed in all 9 patients.The mean duration of the surgical procedure was 59 min,and the average size of the foreign bodies was approximately 26 mm3.Postoperative follow-up exceeding 6 months revealed no complications.Conclusion:The combined use of a surgical navigation system and intraoperative CT represents a potent and effective strategy for the precise localization and subsequent removal of small foreign bodies from the soft tissue structures of the maxillofacial region.This integrative approach appears to increase the success rate of surgical interventions in such cases.

    查看更多>>摘要:Maxillofacial fractures in the pediatric population are generally managed by conservative approaches such as soft diet and medication or semi-invasive procedures namely inter-maxillary fixation or circum-mandibular wiring.These approaches are preferred over any invasive treatment to minimize injury to the growing skeleton and tooth germs.Displaced fractures that cause functional problems such as restricted mouth opening,malocclusion or impaired breathing,mandate open reduction and internal fixation.However,surgical management is associated with morbidity related to general anesthesia,risk of injury to vital structures,and potential,skeletal or dental growth disturbances.This case report de-scribes a non-invasive method of managing displaced,multiple fractures of the mandible in a pediatric patient,with the use of low intensity pulsed ultrasound to achieve favorable clinical outcomes and nil complications.Neither immobilization of the mandible with inter-maxillary fixation nor open reduction and internal fixation was used.Low intensity pulsed ultrasound therapy is painless and patient-friendly.

    Internal iliac artery ligation as a damage control method in hemodynamically unstable pelvic fractures:A systematic review of the literature

    查看更多>>摘要:Purpose:Internal iliac artery ligation(IIAL)has been used as a damage control procedure to treat he-modynamically unstable pelvic fracture for many years.However,there is ongoing debate regarding the effectiveness and safety of this hemostatic method.Therefore,we performed a systematic literature review to assess the efficacy and safety of IIAL for pelvic fracture hemostasis.Methods:Three major databases,PubMed,Embase,and Google Scholar,were searched to screen eligible original studies published in English journals.Two reviewers independently read the titles,abstracts,and full texts of all literature.Articles were included if they reported the use and effects of IIAL.Results:A total of 171 articles were initially identified,with 22 fully meeting the inclusion criteria.Among the analyzed cases,up to 66.7%of patients had associated abdominal and pelvic organ injuries,with the urethra being the most frequently injured organ,followed by the bowel.The outcomes of IIAL for achieving hemostasis in pelvic fractures were found to be satisfactory,with an effective rate of 80%.Hemorrhagic shock was the leading cause of death,followed by craniocerebral injury.Notably,no reports of ischemic complications involving the pelvic organs due to IIAL were found.Conclusion:IIAL has a good effect in treating hemodynamically unstable pelvic fracture without the risk of pelvic organ ischemia.This procedure should be considered a priority for hemodynamically unstable pelvic fracture patients with abdominal organ injuries.

    The cytochrome P4501A1(CYP1A1)inhibitor bergamottin enhances host tolerance to multidrug-resistant Vibrio vulnificus infection

    查看更多>>摘要:Purpose:Vibrio vulnificus(V.Vulnificus)infection is characterized by rapid onset,aggressive progression,and challenging treatment.Bacterial resistance poses a significant challenge for clinical anti-infection treatment and is thus the subject of research.Enhancing host infection tolerance represents a novel infection prevention strategy to improve patient survival.Our team initially identified cytochrome P4501A1(CYP1A1)as an important target owing to its negative modulation of the body's infection tolerance.This study explored the superior effects of the CYP1A1 inhibitor bergamottin compared to antibiotic combination therapy on the survival of mice infected with multidrug-resistant V.Vulnificus and the protection of their vital organs.Methods:An increasing concentration gradient method was used to induce multidrug-resistant V.Vulnificus development.We established a lethal infection model in C57BL/6J male mice and evalu-ated the effect of bergamottin on mouse survival.A mild infection model was established in C57BL/6J male mice,and the serum levels of creatinine,urea nitrogen,aspartate aminotransferase,and alanine aminotransferase were determined using enzyme-linked immunosorbent assay to evaluate the effect of bergamottin on liver and kidney function.The morphological changes induced in the presence of ber-gamottin in mouse organs were evaluated by hematoxylin and eosin staining of liver and kidney tissues.The bacterial growth curve and organ load determination were used to evaluate whether bergamottin has a direct antibacterial effect on multidrug-resistant V.Vulnificus.Quantification of inflammatory factors in serum by enzyme-linked immunosorbent assay and the expression levels of inflammatory factors in liver and kidney tissues by real-time quantitative polymerase chain reaction were performed to evaluate the effect of bergamottin on inflammatory factor levels.Western blot analysis of IκBα,phos-phorylated IκBα,p65,and phosphorylated p65 protein expression in liver and kidney tissues and in human hepatocellular carcinomas-2 and human kidney-2 cell lines was used to evaluate the effect of bergamottin on the nuclear factor kappa-B signaling pathway.One-way ANOVA and Kaplan-Meier analysis were used for statistical analysis.Results:In mice infected with multidrug-resistant V.Vulnificus,bergamottin prolonged survival(p=0.014),reduced the serum creatinine(p=0.002),urea nitrogen(p=0.030),aspartate amino-transferase(p=0.029),and alanine aminotransferase(p=0.003)levels,and protected the cellular morphology of liver and kidney tissues.Bergamottin inhibited interleukin(IL)-1 p,IL-6,and tumor ne-crosis factor(TNF)-α expression in serum(IL-1β:p=0.010,IL-6:p=0.029,TNF-α:p=0.025)and inhibited the protein expression of the inflammatory factors IL-1β,IL-6,TNF-α in liver(IL-1β:p=0.010,IL-6:p=0.011,TNF-α:p=0.037)and kidney(IL-1p:p=0.016,IL-6:p=0.011,TNF-α:p=0.008)tissues.Bergamottin did not affect the proliferation of multidrug-resistant V.Vulnificus or the bacterial load in the mouse peritoneal lavage fluid(p=0.225),liver(p=0.186),or kidney(p=0.637).Conclusion:Bergamottin enhances the tolerance of mice to multidrug-resistant V.Vulnificus infection.This study can serve as a reference and guide the development of novel clinical treatment strategies for V.Vulnificus.

    Intramedullary nailing for irreducible spiral subtrochanteric fractures:A comparison of cerclage and non-cerclage wiring

    查看更多>>摘要:Purpose:Intramedullary nailing is the preferred internal fixation technique for the treatment of sub-trochanteric fractures because of its biomechanical advantages.However,no definitive conclusion has been reached regarding whether combined cable cerclage is required during intramedullary nailing treatment.This study is performed to compare the clinical effects of intramedullary nailing with cerclage and non-cerclage wiring in the treatment of irreducible spiral subtrochanteric fractures.Methods:Patients with subtrochanteric fractures admitted to our center from January 2013 to December 2021 were retrospectively analyzed.The patients were enrolled in the case-control study according to the inclusion and exclusion criteria and divided into the non-cerclage group and the cerclage group.The patients'clinical data,including the operative time,intraoperative blood loss,hospital stay,reoperation rate,fracture union time,and Harris hip score,were compared between these 2 groups.Categorical variables were compared using Chi-square or Fisher's exact test.Continuous variables with normal distribution were presented as mean±standard deviation and analyzed with Student's t-test.Non-normally distributed variables were expressed as median(Qi,Q3)and assessed using the Mann-Whitney test.A p<0.05 was considered significant.Results:In total,69 patients were included in the study(35 patients in the non-cerclage group and 34 patients in the cerclage group).The baseline data of the 2 groups were comparable.There were no significant difference in the length of hospital stay(z=-0.391,p=0.696),operative time(z=-1.289,p=0.197),or intraoperative blood loss(z=-1.321,p=0.186).However,compared with non-cerclage group,the fracture union time was shorter(z=-5.587,p<0.001),the rate of nonunion was lower(x2=6.030,p=0.03),the anatomical reduction rate was higher(x2=5.449,p=0.03),and the Harris hip score was higher(z=-2.99,p=0.003)in the cerclage group,all with statistically significant differences.Conclusions:Intramedullary nailing combined with cable cerclage wiring is a safe and reliable technique for the treatment of irreducible subtrochanteric fractures.This technique can improve the reduction effect,increase the stability of fracture fixation,shorten the fracture union time,reduce the occurrence of nonunion,and contribute to the recovery of hip joint function.
