目的:观察补肾活血汤合五苓散对阳虚血瘀型射血分数降低性心力衰竭(Heart failure with reduced ejection fraction,HFrEF)患者的心功能及hs-CRP、hs-cTnI水平的影响.方法:纳入阳虚血瘀型HFrEF患者100例,随机分成观察组和对照组,每组50例.对照组给予常规西药治疗,观察组在常规治疗基础上加用补肾活血汤合五苓散,治疗4周后观察比较2组患者治疗前后的心功能及血清BNP、hs-CRP、hs-cTnI水平.结果:2组患者治疗后的NYHA心功能分级、LVEF、6MWD、BNP、hs-CRP水平均优于治疗前,差异有统计学意义;观察组患者的NYHA心功能分级、LVEF、6MWD、BNP的治疗后水平均优于对照组,差异有统计学意义;2组患者hs-CRP和hs-cTnI的治疗后水平组间比较,差异无统计学意义.结论:在常规西医治疗的基础上加用补肾活血汤合五苓散能够更好地改善阳虚血瘀型HFrEF患者的心功能分级、LVEF、6MWD及BNP水平,但对患者hs-CRP、hs-cTnI水平无明显影响.
Clinical Observation on the Treatment of Heart Failure with Reduced Ejection Fraction of Yang Deficiency and Blood Stasis Type by Bushen Huoxue Decoction and Wuling Powder
Objective: To observe the effect of Bushen Huoxue Decoction combined with Wuling Powder on cardiac function and hs-CRP and hs-cTnI levels in patients with heart failure with reduced ejection fraction (HFrEF) of Yang deficiency and blood stasis type. Methods: 100 HFrEF patients with Yang deficiency and blood stasis type were en-rolled and randomly divided into observation group and control group, 50 cases in each group. The control group was given conventional western medicine treatment, and the observation group was given Bushen Huoxue Decoction and Wuling Powder in addition to conventional treatment. After 4 weeks of treatment, the cardiac function and serum BNP, hs-CRP, and hs-cTnI levels of the two groups of patients before and after treatment were observed and compared. . Re-sults: The NYHA cardiac function class, LVEF, 6MWD, BNP, and hs-CRP levels of the patients in the 2 groups after treatment were all higher than those before treatment, and the differences were statistically significant; the NYHA car-diac function class, LVEF, 6MWD, and BNP levels of the patients in the observation group were higher than those be-fore treatment. The levels after treatment were better than those in the control group, and the difference was statistically significant; the post-treatment levels of hs-CRP and hs-cTnI in the two groups of patients were compared between the groups, and the difference was not statistically significant. Conclusion: On the basis of conventional Western medicine treatment, the addition of Bushen Huoxue Decoction and Wuling Powder can better improve the cardiac function clas-sification, LVEF, 6MWD and BNP levels in patients with HFrEF of Yang deficiency and blood stasis type, but it has no effect on hs-CRP, hs -cTnI levels had no significant ef-fect.
