摘要
目的:探究中药穴位贴敷联合耳穴埋豆对强直性脊柱炎患者PSQI评分及疼痛程度的影响.方法:选取河南中医药大学第一附属医院2022年1月—2023年1月AS患者98例,按照随机数字表法分为2组,各49例.常规治疗组采取塞来昔布常规治疗,并指导患者脊柱锻炼.中医联合组在常规治疗组的治疗方案基础上加以中药穴位贴敷联合耳穴埋豆干预.观察PSQI评分、脊柱活动度、腰背疼痛、中医证候积分及生活质量变化.结果:治疗前2组PSQI评分、脊柱活动度比较,差异无统计学意义(P>0.05);治疗14d后2组PSQI评分低于治疗前,脊柱活动度>治疗前,且中医联合组PSQI评分低于常规治疗组,脊柱活动度>常规治疗组(P<0.05).治疗前2组腰背疼痛程度比较差异无统计学意义(P>0.05);治疗14d后2组腰背疼痛程度轻于治疗前,且中医联合组腰背疼痛程度轻于常规治疗组(P<0.05).治疗前2组晨僵、关节肿痛、身体疲乏积分比较差异无统计学意义(P>0.05);治疗14d后2组晨僵、关节肿痛、身体疲乏积分低于治疗前,且中医联合组晨僵、关节肿痛、身体疲乏积分低于常规治疗组(P<0.05).治疗前2组EASi-Qol评分比较,差异无统计学意义(P>0.05);治疗14d后2组EASi-Qol评分低于治疗前,且中医联合组EASi-Qol评分低于常规治疗组(P<0.05).结论:中药穴位贴敷联合耳穴埋豆能够改善AS患者临床症状及脊柱活动度,减轻其腰背疼痛程度,进一步改善患者睡眠质量及生活质量.
Abstract
Objective: To explore the effect of traditional Chinese medicine acupoint application combined with ear-point bean embedding on PSQI scores and pain levels in patients with ankylosing spondylitis. Methods: 98 AS patients from the First Affiliated Hospital of Henan University of Traditional Chinese Medicine from January 2022 to Janu-ary 2023 were selected and divided into 2 groups according to the random number table method, with 49 cases in each group. The conventional treatment group received conventional treatment with celecoxib and guided spinal exercises. On the basis of the treatment plan of the conventional treatment group, the combined traditional Chinese medicine group added traditional Chinese medicine acupoint application combined with ear-point buried bean intervention. Changes in PSQI score, spinal mobility, low back pain, TCM syndrome scores and quality of life were observed. Re-sults: There was no statistically significant difference in PSQI scores and spinal mobility between the two groups before treatment (P>0.05); after 14 days of treatment, the PSQI scores of the two groups were lower than before treatment, and the spinal mobility was greater than before treatment, and the PSQI score of the combined traditional Chinese medi-cine group was lower than before treatment. It was lower than that of the conventional treatment group, and the spinal mobility was greater than that of the conventional treatment group (P<0.05). There was no significant difference in the degree of back pain between the two groups before treatment (P>0.05); after 14 days of treatment, the degree of back pain in the two groups was lighter than before treatment, and the degree of back pain in the combined traditional Chi-nese medicine group was lighter than that in the conventional treatment group (P<0.05). There was no statistically sig-nificant difference in morning stiffness, joint swelling and pain, and body fatigue scores between the two groups before treatment (P>0.05); after 14 days of treatment, morning stiffness, joint swelling and pain, and body fatigue scores in the two groups were lower than before treatment, and the combined traditional Chinese medicine group Morning stiffness, joint swelling and pain, and physical fatigue scores were lower than those in the conventional treatment group (P<0.05). Comparing the EASi-Qol scores of the two groups before treatment, the difference was not statistically significant (P>0.05); after 14 days of treatment, the EASi-Qol scores of the two groups were lower than before treatment, and the EASi-Qol score of the combined traditional Chinese medicine group was lower than that of the conventional treatment group (P<0.05). Conclusion: Traditional Chinese medicine acupoint application combined with ear-point buried beans can improve the clinical symptoms and spinal mobility of AS patients, reduce their back pain, and further improve the patients' sleep quality and life quality.
