目的:观察归脾汤联合穴位贴敷对心脾两虚型室性早搏患者危险分层的影响.方法:选择2022年1—10月在南京中医药大学太仓附属医院心病科住院部或门诊部就诊的符合纳入标准的心脾两虚型室性早搏患者共74例作为研究对象,将其随机分为治疗组和对照组各37例.对照组给予生活方式干预及现代医学标准抗心律失常治疗,治疗组在对照组治疗基础上加用归脾汤和穴位贴敷,疗程均为8周,观察2组患者治疗前后中医证候积分及疗效、室性早搏总数、Lown分级、室早指数、室早易损指数、室性早搏QRS-T波群形态以及安全性指标的变化情况.结果:治疗后,2组中医证候总积分均明显下降,治疗组积分下降情况优于对照组;治疗组的中医证候有效率显著高于对照组;2组24h室性早搏总数均低于治疗前;治疗组Lown分级分布情况优于治疗前,且优于对照组;治疗组室早指数改善显著且优于对照组;治疗组室早易损指数改善显著且优于对照组;治疗组室早QRS-T波群形态优化情况优于对照组,差异均有统计学意义.治疗期间2组均未出现明显不良反应.结论:归脾汤联合穴位贴敷能有效改善心脾两虚型室性早搏患者的中医证候,提高疗效,降低危险分层,且安全性良好.
Clinical Observation of Guipi Decoction Combined with Acupoint Application in Reducing the Risk Stratification of Ventricular Premature Beat
Objective: To observe the effect of Guipi decoction combined with acupoint application on risk stratifica-tion in patients with premature ventricular contractions of heart and spleen deficiency type. Methods: A total of 74 patients with heart and spleen deficiency type premature ventricular contractions who met the inclusion criteria and were admit-ted to the inpatient or outpatient department of the Department of Cardiology, Taicang TCM Hospital Affiliated to Nanjing University of Chinese Medicine from January to October 2022 were selected as research subjects, and they were randomly divided into There were 37 cases in each treatment group and control group. The control group was given lifestyle inter-vention and modern medical standard anti-arrhythmic treatment, while the treatment group was given Guipi decoction and acupoint application on the basis of the treatment in the control group. The course of treatment was 8 weeks in both groups. The TCM syndrome scores and scores of the patients in the two groups were observed before and after treatment. Changes in efficacy, total number of premature ventricular contractions, Lown grade, premature ventricular contraction index, pre-mature ventricular vulnerability index, QRS-T wave complex morphology of premature ventricular contractions, and safety indicators. Results: After treatment, the total scores of TCM syndromes in both groups were significantly reduced, and the score reduction in the treatment group was better than that in the control group; the effective rate of TCM syndromes in the treatment group was significantly higher than that in the control group; the total number of premature ventricular con-tractions in 24 hours in both groups was lower than Before treatment; the Lown grade distribution of the treatment group was better than before treatment and better than that of the control group; the premature ventricular index of the treatment group improved significantly and was better than that of the control group; the premature ventricular vulnerability index of the treatment group improved significantly and was better than that of the control group; the treatment group The opti-mization of ventricular premature QRS-T wave complex morphology was better than that of the control group, and the dif-ferences were statistically significant. No obvious adverse reactions occurred in either group during the treatment period. Conclusion: Guipi decoction combined with acupoint application can effectively improve the traditional Chinese medicine syndrome in patients with premature ventricular contractions due to heart and spleen deficiency, improve efficacy, reduce risk stratification, and has good safety.
