Clinical Experience of Chu Haoran in Treating with Modified wendan Decoction
Gastroesophageal reflux disease (GERD) is a prevalent chronic disorder of the digestive tract, falling under TCM categories such as "acid regurgitation," "plum qi," "stomachache," "chest pain," and "vomiting." Its incidence has been steadily increasing over the years, significantly impacting individuals' physical and mental well-being. Professor Chu Hao-ran has dedicated over 30 years to clinical and fundamental research, specializing in the treatment of gastrointestinal diseases. He posits that the pathogenesis of GERD lies in spleen and stomach weakness, with phlegm-dampness obstruction being a crucial pathological factor. Additionally, disharmony between bile and stomach commonly manifests clinically. Spleen-stomach weakness leads to impaired upward movement of spleen en-ergy and downward movement of stomach energy, particularly imbalancing the stomach function. Dysfunction in spleen ascending and stomach descending disrupts middle jiao equilibrium, resulting in water-fluid metabolism disorders along with phlegm-dampness accumulation that obstructs qi flow while causing inverse movements of spleen-stomach functions. Deficiency in spleen-stomach function leads to stagnation within soil-wood elements; liver depression affects bile circulation while reversing its normal flow with gastric qi. This ultimately results in discomfort characterized by burning sensation behind sternum, acid regurgitation, and belching. Therefore, treatment for gastroesophageal reflux disease focuses on reducing gastric refluxion while resolving phlegm accumulation through regulating qi flow. Strength-ening spleen-stomach function restores transportation and absorption capabilities while balancing middle jiao functions to eliminate phlegm-dampness obstruction alongside regulating overall qi mechanism. This paper extensively collects original medical cases from Professor Chu Hao-ran's practice for studying purposes. It summarizes his clinical expe-rience regarding GERD treatment using Jiawei
