摘要
李雪苓教授经过数十年教学及临床工作,认为高血压合并失眠,其病机属于本虚标实,肝肾阴虚,心肾不交,神失所养导致.发病主要与心、肝、肾三脏相关;辨证治疗上予"养心安神,肝肾同调"为法;选用古方上下两济汤及归芍地黄汤进行加减治疗,对高血压合并失眠患者伴随兼症,李教授提出辨脉为主,症状为辅的临证思路.总结出对治角药,运用灵活,疗效显著.辨脉加用角药,脉弦-柴胡-陈皮-川芎、脉细-女贞子-桑葚-黄芪、脉沉-桑寄生-杜仲-枸杞、脉滑-白术-茯苓-半夏.文章结合相关专业文献,将李雪苓教授治疗高血压合并失眠的选方及角药予总结,辅以临床药理研究分析,体现中医药辨证论治有效及优势,以供临床参考.
Abstract
After decades of teaching and clinical work,Professor Li Xueling believes that the pathogenesis of hy-pertension combined with insomnia is deficiency in origin and excess in excess,liver and kidney yin deficiency,heart and kidney disharmony,and loss of mental support.The disease is mainly related to the three internal organs of the heart,liver,and kidneys;the method of syndrome differentiation and treatment is"nourishing the heart and calming the nerves,and harmonizing the liver and kidneys";the ancient prescriptions Shangxia Liangji Decoction and Guishao Dihuang Decoction are used for modified treatment to treat patients with hypertension and insomnia.With concurrent symptoms,Professor Li proposed a clinical approach based on pulse identification and supplemented by symptoms.It is concluded that anti-angular medicines are flexible in application and have significant curative effect.Angle medicine is used to diagnose the pulse,including stringy pulse-Bupleurum-dried tangerine peel-Ligusticum chuanxiong,thin pulse-Ligustrum lucidum-mulberry-astragalus,deep pulse-mulberry-Eucommia ulmoides-wolfberry,slippery pulse-Atractylodes-Poria-Pinellia ternata.This article combines relevant professional literature to sum-marize Professor LI Xueling's selection of prescriptions and medicinal herbs for treating hypertension combined with insomnia,supplemented by clinical pharmacological research and analysis,which reflects the effectiveness and advan-tages of traditional Chinese medicine syndrome differentiation and treatment for clinical reference.
