杨廉方基于肝脾同调论治癌性呃逆的临证精粹

摘要
癌性呃逆为肿瘤患者经历的常见的病症之一,目前西医学对该病的致病因素和发生机制尚未明确,治疗上常选用甲氧氯普胺、巴氯芬等药物治疗,但在长期使用后易产生复发性和耐药性.杨廉方教授系重庆市名中医,并担任第五批全国老中医药专家学术经验传承工作的指导老师,擅长运用中医药治疗肿瘤、脾胃病等疾病.杨廉方教授认为脾阳不足、肝气犯胃为发生癌性呃逆的关键病因,其病机主要为肝脾失和.强调癌性呃逆日久易"痰"易"湿"易"瘀",故当明确标本,辨明虚实,治疗当分清轻重缓急,并兼顾受扰之邪,为临床治疗癌性呃逆提供了新思路.附验案1 例以佐证.
Abstract
Cancerous hiccup is one of the common symptoms experienced by cancer patients.At present,Western medicine has not yet clear the causative factors and mechanisms of the disease.Metoclopramide,baclofen and other drugs are often used for treatment.However,in Relapse and drug resistance are likely to occur after long-term use.Professor YANG Lianfang is a famous Chinese medicine doctor in Chongqing and serves as the instructor for the fifth batch of national senior Chinese medicine experts to inherit their academic experience.He is good at using Chinese medicine to treat tumors,spleen and stomach diseases and other diseases.Professor YANG Lianfang believes that in-sufficient spleen yang and liver Qi invading the stomach are the key causes of cancerous hiccups,and the main patho-genesis is disharmony between the liver and spleen.It is emphasized that cancerous hiccups are prone to"phlegm","dampness"and"blood stasis"over time,so the specimens should be clearly identified,deficiency and excess should be distinguished,and treatment should prioritize and take into account the disturbed evils,which provides a new idea for the clinical treatment of cancerous hiccups.Attached is a case study to support it.
