摘要
风湿性多肌痛患者常以快速解决"颈、肩、骨盆带肌的晨僵、疼痛"为首要诉求,病情反复发作,严重影响患者身心健康.温伟强主任医师认为该病核心病机为脾肾阳虚,肾阳虚则无以充养温煦筋骨,脾虚失健运,气血不足,四肢百骸无以濡养,"不荣则痛";脾升胃降为一身气机升降之枢,脾运化水液失职,内生痰湿,痰湿阻络;脾失健运,肝失疏泄,气滞血瘀,脉络瘀阻而为痹;日久则痰湿瘀潜伏于内,为病情反复发作之根源.治疗上重视透邪之法,以清伏邪,治以益肾健脾、活血透邪,临床疗效显著.
Abstract
Patients with polymyalgia rheumatica often seek quick relief from"morning stiffness and pain in the neck,shoulders,and pelvic girdle muscles"as their primary complaint.The condition relapses,seriously affecting the patient's physical and mental health.Chief Physician Wen Weiqiang believes that the core pathogenesis of the disease is spleen and kidney yang deficiency.Deficiency of kidney yang means that the muscles and bones cannot be nourished.Deficiency of the spleen will lead to poor health,insufficient Qi and blood,and the limbs cannot be nourished."With-out prosperity,there will be pain";The spleen ascends and the stomach descends as the pivot of the whole body's Qi movement.The spleen fails to transport and transform water,resulting in phlegm and dampness,which blocks the col-laterals.The spleen fails to move well,the liver fails to relieve diarrhea,Qi stagnation and blood stasis,and venous sta-sis are blocked,resulting in paralysis.As time goes by,phlegm,dampness and blood stasis lurk inside,which is the root cause of recurring disease.In treatment,emphasis is placed on the method of clearing away evil spirits,nourishing the kidneys,strengthening the spleen,activating blood and clearing away evil spirits.The clinical efficacy is remarkable.
