试谈分期辨证治疗过敏性紫癜性肾炎

摘要
过敏性紫癜性肾炎属本虚标实证,禀赋不足为其本,外感风湿热毒为其标,瘀热阻络为发病之关键,湿热伤肾为病情进展重要因素.临证以分期论治,初期以风热搏结、血热伤络为主,治以祛风清热、凉营化瘀;中期邪气渐退,湿热耗气伤阴,以气阴两虚为主,治以益气养阴、清利和络;后期正气亏虚,邪气留恋,治以补肾健脾、化湿泄浊;同时重视生活的预防调摄.文章主要从病因病机、分期论治及典型案例等方面介绍过敏性紫癜性肾炎的治疗.
Abstract
Henoch-Schonlein purpura nephritis is characterized by a deficiency in the root cause and a syndrome in which the root cause is insufficient endowment.Exogenous rheumatism and heat are the symptoms.Blood stasis and heat blocking the collaterals are the key to the disease.Damp-heat damage to the kidneys is an important factor in the progression of the disease.Clinical symptoms are treated in stages.In the early stage,wind-heat fights knots and blood-heat damages the collaterals.Treatment is to dispel wind,clear away heat,and cool the camp to remove blood stasis.In the middle stage,evil Qi gradually recedes,and damp-heat consumes Qi and damages Yin.Deficiency of both Qi and yin is the main cause,the treatment is to replenish Qi and nourish Yin,cleanse and harmonize the collaterals;in the later stage,when the righteousness is deficient and the evil Qi is lingering,the treatment is to nourish the kidney and spleen,remove dampness and release turbidity;at the same time,pay attention to the prevention and adjustment of life.The article mainly introduces the treatment of Henoch-Schonlein purpura nephritis from the aspects of etiology,pathogenesis,stage treatment and typical cases.
