桂枝乌苓方加减内服外熨法治疗中风后痰瘀阻络型肩手综合征的临床观察

摘要
目的:观察桂枝乌苓方加减内服外熨法治疗中风后痰瘀阻络型肩手综合征的临床效果.方法:选取60例诊断为中风后痰瘀阻络型肩手综合征的患者为观察对象,随机分成对照组和观察组.2组均接受常规治疗,观察组加用桂枝乌苓方加减内服外熨法治疗,疗程为4周,评估治疗前后2组的疼痛评分量表(VAS)、上肢功能量表(FMA)、日常活动能力量表(MBI)及中医症候积分的变化.结果:观察组和对照组各自前、后VAS、FMA、MBI、中医症候积分比较,P=0.000＜0.05,观察组计分均优于对照组,表明2组治疗都趋向好转,而观察组好转程度较对照组明显.2组均治疗有效,观察组总有效率93.33％与对照组总有效率73.33％比较明显增高,差异有统计学意义(P＜0.05).结论:在常规治疗的基础上,桂枝乌苓方加减内服外熨法治疗中风后痰瘀阻络型肩手综合征患者能提高疗效,缓解疼痛,改善上肢功能及日常活动能力.
Abstract
Objective:To observe the clinical effect of Guizhi Wuling decoction plus internal and external ironing in the treatment of shoulder-hand syndrome of phlegm and blood stasis type after stroke.Methods:A total of 60 patients diagnosed with shoulder-hand syndrome of phlegm and blood stasis type after stroke were selected as the observa-tion subjects and randomly divided into a control group and an observation group.Both groups received conventional treatment,and the observation group was treated with Guizhi Wuling decoction plus internal and external ironing for 4 weeks.The changes of pain scale(VAS),upper limb function scale(FMA),daily activity scale(MBI)and TCM symp-tom score of the two groups before and after treatment were evaluated.Results:The comparison of VAS,FMA,MBI and TCM symptom score before and after treatment between the observation group and the control group was P=0.000＜0.05.The scores of the observation group were better than those of the control group,indicating that both groups tended to improve,and the improvement of the observation group was more obvious than that of the control group.The treat-ment was effective in both groups.The total effective rate of the observation group was 93.33％,which was significantly higher than that of the control group(73.33％),and the difference was statistically significant(P＜0.05).Conclusion:On the basis of conventional treatment,the Guizhi Wuling decoction plus or minus internal and external ironing method can improve the efficacy,relieve pain,and improve upper limb function and daily activities in patients with post-stroke phlegm and blood stasis type shoulder-hand syndrome.
