Relationship Between Pathogenesis of "Kidney Deficiency and Blood Stasis" and Autophagy Dysfunction in Elderly Patients with Chronic Kidney Disease Based on the Theory of Collateral Diseases
Drawing upon the traditional Chinese medicine(TCM) theory of collateral diseases, this study integrates contemporary clinical and basic research findings to posit that the primary pathological mechanism underlying chronic kidney disease (CKD) in the elderly is "kidney deficiency and collateral stasis." As individuals age, kidney qi wanes, compromising its transformative functions, leading to the accumulation of dampness-turbidity and blood stasis that obstructs the kidney collaterals. This pathological cascade mirrors the reduced autophagy and inefficient clearance of pathological products seen during the aging process, both ultimately contributing to kidney damage. The therapeutic strategy of tonifying kidney essence and dredging the collaterals aims to protect the kidneys by modulating autophagy, thereby slowing the progression of CKD in the elderly. By delving into the scientific essence of "kidney deficiency and collateral stasis" in elderly CKD,this study offers fresh perspectives and robust scientific foundations for the TCM treatment and underlying mechanisms of this condition.
