摘要
目的:研究运动疗法联合九味固金胶囊对慢性阻塞性肺疾病(COPD)稳定期肺肾两虚型患者肺功能的影响.方法:将山西省中医院符合标准的114例COPD稳定期肺肾两虚型患者按随机数字表法分为A组:运动训练+九味固金胶囊组(n=27)、B组:九味固金胶囊组(n=29)、C组:运动训练组(n=28)、D组:基础治疗组(n=30).A、B、C组在D组基础上分别加用运动训练联合九味固金胶囊、九味固金胶囊、运动训练,治疗周期为1月,观察各组肺功能指标(FEV1/FVC%、FEV1%)、动脉血气指标(PaO2、PaCO2).结果:与治疗前相比,4组肺功能(FEV1/FVC%、FEV1%)、PaO2、A组PaCO2均有改善,差异具有统计学意义(P<0.01),B组、C组、D组PaCO2治疗前后无明显差异(P>0.05).与D组相比,治疗后A组肺功能(FEV1/FVC%、FEV1%)、PaO2显著改善,差异具有统计学意义(P<0.05).治疗后B组与C组相比肺功能(FEV1/FVC%、FEV1%)无明显差异(P>0.05),而PaO2有显著差异,C组强于B组.九味固金胶囊和运动训练对于改善改善肺功能(FEV1/FVC%、FEV1%),二者交互作用不显著(P>0.05),合用不如单用运动训练,对于改善PaO2,二者交互作用显著(P<0.05),联合使用效果最好.结论:运动疗法联合九味固金胶囊可以较好改善COPD稳定期肺肾两虚型患者的肺功能.
Abstract
Objective:To study the effect of exercise therapy combined with Jiuwei Gujin capsule on lung function in patients with lung and kidney deficiency in stable COPD. Methods:A total of 114 patients with lung and kidney de-ficiency in stable COPD who met the criteria in Shanxi Provincial Hospital of Traditional Chinese Medicine were ran-domly divided into group A:exercise training+Jiuwei Gujin capsule group (n=27),group B:Jiuwei Gujin capsule group (n=29),group C:exercise training group (n=28),and group D:basic treatment group (n=30). Groups A,B,and C were treated with exercise training combined with Jiuwei Gujin capsule,Jiuwei Gujin capsule,and exercise training,respec-tively,on the basis of group D. The treatment cycle was 1 month,and the lung function indexes (FEV1/FVC%,FEV1%) and arterial blood gas indexes (PaO2,PaCO2) were observed in each group. Results:Compared with before treatment,the lung function (FEV1/FVC%,FEV1%),PaO2 of the four groups and PaCO2 of group A were improved,and the dif-ference was statistically significant (P<0.01). There was no significant difference in PaCO2 before and after treatment in groups B,C and D (P>0.05). Compared with group D,the lung function (FEV1/FVC%,FEV1%) and PaO2 of group A were significantly improved after treatment,and the difference was statistically significant (P<0.05). After treatment,there was no significant difference in lung function (FEV1/FVC%,FEV1%) between group B and group C (P>0.05),but there was a significant difference in PaO2,and group C was stronger than group B. Jiuwei Gujin capsule and exer-cise training improved lung function (FEV1/FVC%,FEV1%),and the interaction between the two was not significant (P>0.05). The combined use was not as good as exercise training alone. For improving PaO2,the interaction between the two was significant (P<0.05),and the combined use had the best effect. Conclusion:Exercise therapy combined with Jiuwei Gujin capsule can effectively improve the lung function of patients with lung and kidney deficiency in the stable stage of COPD.
