摘要
目的:基于R语言数据挖掘的方法探索中药治疗类风湿关节炎(RA)的临床证候及组方用药规律.方法:检索中国知网、万方和维普等数据库从建库起至 2023 年 10 月有关中药治疗RA的临床研究文献.采用频次分析、相关性分析、关联规则分析、聚类分析等方法进行统计分析.结果:最终纳入方剂 344 首.该病有风寒痹阻证、湿热痹阻证、肝肾不足证等 8 个证型,共使用单味中药 251 味,累计使用 4066 次,使用频次前 5 的依次是甘草、当归、桂枝、白芍和黄芪.使用频次>50 次的中药有 30 味,涉及 13 种功效,以发散风寒药、补血药、补气药和祛风寒湿药为主.相关性分析得到相关系数高的 20 种药物组合,关联规则分析得到羌活→独活和羌活→独活、桂枝等以独活为核心的药物配伍.聚类分析得到 2 个新方组合,针对不同证型可辨证论治.结论:该研究明确了中医治疗RA以益气养血,补益肝肾为主,祛风除湿止痛为辅的用药特点,可为RA的中医用药提供参考和借鉴.
Abstract
Objective:To explore the clinical syndrome and prescription rules of Chinese medicine in the treatment of rheumatoid arthritis(RA)based on R language data mining.Methods:The clinical research literature on Chinese medicine treatment of RA from the establishment of CNKI,Wanfang and VIP databases to October 2023 was retrieved.Frequency analysis,correlation analysis,association rule analysis,cluster analysis and other methods were used for statistical analysis.Results:Finally,344 prescriptions were included.The disease has 8 syndromes,including wind-cold arthralgia syndrome,damp-heat arthralgia syndrome,liver-kidney deficiency syndrome,etc.A total of 251 single Chinese medicines were used,with a total of 4066 times.The top 5 in terms of frequency of use were licorice,angelica,cinnamon twig,white peony root and astragalus.There were 30 Chinese medicines with a frequency of use>50 times,involving 13 effects,mainly dispersing wind-cold drugs,blood-tonifying drugs,qi-tonifying drugs and wind-cold-dampness-dispelling drugs.Correlation analysis obtained 20 drug combinations with high correlation coefficients,and association rule analysis obtained drug combinations with Duhuo as the core,such as Qianghuo → Duhuo and Qianghuo → Duhuo,Guizhi.Cluster analysis obtained 2 new prescription combinations,which can be used for syn-drome differentiation and treatment according to different syndrome types.Conclusion:This study clarified the char-acteristics of TCM treatment of RA,which is mainly to nourish qi and blood,nourish the liver and kidney,and to dispel wind,dampness and relieve pain,which can provide reference and reference for TCM medication of RA.
