盆腔炎性疾病后遗症(sequelae of pelvic inflammatory disease,SPID)是当今困扰我国女性生育、健康及日常生活的常见病、多发病.是由于盆腔炎性疾病(pelvic inflammatory disease,PID)未经过正确规范的诊治,迁延不愈而导致盆腔内输卵管、卵巢、盆腔结缔组织以及腹膜的慢性炎症,表现为慢性盆腔痛、不孕以及盆腔粘连.SPID后期多表现为无菌性炎症,长期炎症刺激引起组织破坏、粘连、增生以及瘢痕形成,因此控制炎症反应是治疗该病的主要方法.西医对于该病的治疗以经验性使用抗生素为主,但长期应用易产生耐药性且不良反应大,无法根除病因.中医学多认为该病与外感寒湿、内生湿热、久病体虚或情志不畅有关.传统中医药在治疗该病中发挥了独特优势,其抗炎效果显著,治疗方法包括:辨证论治的中医内治法,以其多靶点多途径的特点发挥作用;以及中医外治法,包括中药保留灌肠、中药穴位贴敷、中药离子导入、针灸和多种疗法联合治疗等.其抗炎作用机制主要与改善血液流变学、调节细胞因子表达、抗氧化应激有关.作者查阅文献后发现,中医药治疗PID已成为新的趋势,但尚缺乏对中医药治疗方法和作用机制的系统阐述.故作者通过查阅近年相关文献,对SPID的中医病因病机,中医药治疗方法及作用机制的研究进行归纳总结,以期为后续中医药防治SPID的临床研究提供一定的理论依据.
overview of the Progress and Mechanism of TCM treatment of Sequelae of Pelvic Inflammatory Disease
The sequelae of pelvic inflammatory disease(SPID)is a common and frequently-occurring disease that plagues women's fertility,health and daily life in my country today.Pelvic inflammatory disease(PID)is caused by chronic inflammation of the fallopian tubes,ovaries,pelvic connective tissue and peritoneum in the pelvis without cor-rect and standardized diagnosis and treatment,resulting in chronic inflammation of the fallopian tubes,ovaries,pelvic connective tissue and peritoneum,manifesting as chronic pelvic pain,infertility and pelvic Adhesions.The late stage of SPID mostly manifests as sterile inflammation.Long-term inflammatory stimulation causes tissue destruction,ad-hesion,hyperplasia,and scar formation.Therefore,controlling the inflammatory response is the main method to treat the disease.Western medicine mainly focuses on the empirical use of antibiotics for the treatment of this disease,but long-term use is prone to drug resistance and has serious side effects,and the cause cannot be eradicated.Traditional Chinese medicine believes that this disease is related to exogenous cold and dampness,endogenous dampness and heat,chronic illness,physical weakness,or poor mood.Traditional Chinese medicine has played a unique advantage in the treatment of this disease,and its anti-inflammatory effect is significant.Treatment methods include:internal TCM treatment based on syndrome differentiation and treatment,which has the characteristics of multiple targets and mul-tiple pathways;and external TCM treatment,including Chinese medicine retention enema,Chinese medicine acupoint application,Chinese medicine iontophoresis,acupuncture and a variety of combined therapies.Its anti-inflammatory mechanism is mainly related to improving blood rheology,regulating cytokine expression,and resisting oxidative stress.After reviewing the literature,the author found that TCM treatment of PID has become a new trend,but there is still a lack of systematic elaboration of TCM treatment methods and mechanisms of action.Therefore,the author reviewed the relevant literature in recent years and summarized the research on the TCM etiology and pathogenesis of SPID,TCM treatment methods and mechanisms of action,in order to provide a certain theoretical basis for subsequent clinical re-search on TCM prevention and treatment of SPID.
