Effect of Qingshen Granule on Blood MicroRNA-21 ,Wnt4 ,and β-catenin in Patients with Non-dialysis Chronic Kidney Disease with Damp-Heat Syndrome
Objective To investigate the effect of Qingshen Granule on blood microRNA-21,Wnt4,and β-catenin in patients with non-dialysis chronic kidney disease (CKD)with damp-heat syndrome,as well as its mechanism of action in protecting re-nal function and improving clinical symptoms.Methods A total of 66 CKD patients with damp-heat syndrome who met the in-clusion criteria were enrolled,and finally 64 patients completed the study,with 32 patients in the control group and 32 in the observation group.The patients in the control group received routine basic treatment for CKD,while those in the observation group received oral administration of Qingshen Granule in addition to the treatment in the control group;the course of treatment was 12 weeks for both groups.The two groups were observed and compared in terms of traditional Chinese medi-cine (TCM)symptom score,treatment outcome of TCM syndrome,renal function parameters [blood urea nitrogen(BUN),estimated glomerular filtration rate(eGFR),and serum creatinine(SCr)],microinflammation and nutritional indices [hypersensitive C-re-active protein (hs-CRP),albumin(Alb),and prealbumin(PAB)],and the levels of microRNA-21,Wnt4,and β-catenin in blood.Results After treatment,both groups had a significant reduction in TCM symptom score (P<0.05),and the observa-tion group had a significantly greater reduction than the control group (P<0.05).There was no significant difference in the distribution of clinical outcomes between the two groups (P>0.05).After treatment,the observation group had significant improvements in the levels of BUN,SCr,and eGFR,with significantly greater improvements than the control group (P<0.05);after treatment,both groups had a significant reduction in the serum level of hs-CRP (P<0.05)and significant increa-ses in the levels of Alb and PAB (P<0.05),and the observation group had significantly greater improvements in hs-CRP, Alb,and PAB than the control group (P<0.05).Before treatment,the two groups had significantly higher levels of microR-NA-21,Wnt4,and β-catenin than the normal group (P<0.05),and after treatment,the observation group had significant re-ductions in the levels of microRNA-21,Wnt4,and β-catenin (P<0.05),with significantly greater reductions than the control group (P<0.05).Conclusion Qingshen Granule can improve renal function,microinflammation,and malnutrition and allevi-ate clinical symptoms in CKD patients with damp-heat syndrome,possibly by downregulating the expression level of microR-NA-21 and inhibiting the Wnt/β-catenin signaling pathway.
