Differential diagnostic value of spectral CT enhanced scanning in central lung cancer lesions and atelectasis area
Objective To analyze the differential diagnostic value of spectral CT enhanced scanning in central lung cancer lesions and atelectasis area.Method A total of 117 patients with central lung cancer complicated with atelectasis were selected,and all underwent CT plain scan,spectral CT arterial and venous phase enhanced scanning before surgery.The spectral CT arterial and venous phase related indicators including CT value,noise ratio,and iodine concentration were compared between central lung cancer lesions and atelectasis area.The receiver operating characteristic(ROC)curve was drawn and the area under the curve(AUC)was calculated to evaluate the differential diagnostic value of spec-tral CT enhanced scanning related indicators in central lung cancer lesions and atelectasis area.The Kappa test was used to evaluate the consistency between the results of spectral CT enhanced scanning and pathological examination in the dif-ferential diagnosis of central lung cancer lesions and atelectasis area.Result The CT value and noise ratio of central lung cancer lesions in the arterial and venous phase of spectral CT enhanced scanning were significantly higher than those of the atelectasis area,and the iodine concentration in the arterial and venous phase of spectral CT enhanced scanning were significantly lower than those in the atelectasis area,the differences were statistically significant(P<0.01).Spectral CT enhanced scanning arterial and venous phase related indicators had high differential diagnostic value for central lung cancer lesions and atelectasis area,and venous phase related indicators had better differential diagnostic value for central lung cancer lesions and atelectasis area than arterial phase related indicators.The results of differential diagnosis of cen-tral lung cancer lesions and atelectasis area by spectral CT arterial and venous phase enhanced scanning had high consis-tency with pathological examination results(Kappa=0.812,0.949).The results of CT plain scan in differential diagnostic for central lung cancer lesions and atelectasis area had low consistency with pathological examination results(Kappa=0.359).Conclusion Spectral CT enhanced scanning has high differential diagnostic value for central lung cancer lesions and atelectasis area,and its differential diagnostic results have high consistency with pathological examination results.
