Expression levels and clinical significance of miRNA-21,interleukin-17 and hypoxia-inducible factor-1α in patients with multiple myeloma
Objective To investigate the expression levels and clinical significance of microRNA-21(miRNA-21),in-terleukin-17(IL-17)and hypoxia-inducible factor-1α(HIF-1α)in patients with multiple myeloma.Method A total of 78 multiple myeloma patients and 64 healthy subjects were selected as study group and control group,respectively.Real-time fluores cence quantitative polymerase chain reaction(qPCR)was used to detect miRNA-21 level,and enzyme linked immunosorbent assay was used to detect serum IL-17 and HIF-1α levels.The levels of miRNA-21,IL-17 and HIF-1α in two groups and multiple myeloma patients with different prognosis were compared.The correlation between miRNA-21 level and IL-17,HIF-1α levels in multiple myeloma patients was analyzed by Pearson correlation analysis.Cox propor-tional hazard regression model was used to analyze the prognostic factors of patients with multiple myeloma.The diag-nostic value and prognostic value of miRNA-21,IL-17 and HIF-1α single and combined detection in multiple myeloma were analyzed by receiver operating characteristic(ROC)curve.The survival of multiple myeloma patients with different expressions of miRNA-21,IL-17 and HIF-1α were compared.Result The levels of miRNA-21,IL-17 and HIF-1α in study group were significantly higher than those in control group,and the differences were statistically significant(P<0.01).The levels of miRNA-21,IL-17 and HIF-1α in multiple myeloma patients with poor prognosis were significantly higher than those in patients with good prognosis,and the differences were statistically significant(P<0.01).The level of miRNA-21 was positively correlated with IL-17 and HIF-1α levels in multiple myeloma patients(r=0.447,0.462,P<0.05).miRNA-21,IL-17 and HIF-1α were independent prognostic risk factors in multiple myeloma patients(P<0.05).ROC curve showed that the area under the curve(AUC),sensitivity,specificity,positive predictive value and negative pre-dictive value of miRNA-21,IL-17 and HIF-1α combined detection in the diagnosis of multiple myeloma were higher than those of the three single detection.The AUC,sensitivity,and negative predictive value of miRNA-21,IL-17 and HIF-1αcombined detection to predict the prognosis of multiple myeloma patients were higher than those of the three single detec-tion.The two-year cumulative survival rates of multiple myeloma patients in miRNA-21,IL-17 and HIF-1α high expres-sion group were lower than those in miRNA-21,IL-17 and HIF-1α low expression group,respectively,and the differences were statistically significant(P<0.05).Conclusion The levels of miRNA-21,IL-17 and HIF-1α in multiple myeloma pa-tients are highly expressed,and the level of miRNA-21 is positively correlated with IL-17 and HIF-1α levels.The com-bined detection of the three can better diagnose multiple myeloma and predict prognosis.It is helpful to take effective pre-vention and treatment measures and improve the prognosis of multiple myeloma patients.
