摘要
目的 探讨血清胃功能四项[胃蛋白酶原(PG)Ⅰ、PGⅡ、PGⅠ/PGⅡ(PGR)、胃泌素-17(G-17)]、幽门螺杆菌与早期胃癌的关系.方法 将82例行胃癌筛查人群根据胃镜及病理学检查结果分为早癌组(n=20)和良性组(n=62).比较两组患者的临床特征.早期胃癌发生的影响因素采用Logistic回归分析.PGⅡ、G-17、PG Ⅰ、PGR单独及联合检测对早期胃癌的诊断价值采用受试者工作特征(ROC)曲线及曲线下面积(AUC)进行分析.结果 两组患者的年龄、幽门螺杆菌抗体、胃癌家族史、吸烟史、PGⅡ、G-17、PGⅠ、PGR比较,差异均有统计学意义(P＜0.05).Logistic回归分析结果显示,年龄、幽门螺杆菌抗体、胃癌家族史、吸烟史、PGⅠ、PGⅡ、PGR、G-17是早期胃癌发生的影响因素(P＜0.05).ROC曲线显示,PG Ⅰ、PGⅡ、PGR、G-17联合检测诊断早期胃癌的AUC高于各项指标单独检测.结论 年龄、幽门螺杆菌抗体、胃癌家族史、吸烟史、PGⅠ、PGⅡ、PGR、G-17均是早期胃癌发生的影响因素;PGⅠ、PGⅡ、PGR、G-17联合检测对早期胃癌的诊断价值较高.
Abstract
Objective To explore the relationship between four items of serum gastric function[pepsinogen(PG)Ⅰ,PGⅡ,PGⅠ/PGⅡ(PGR),gastrin-17(G-17)],helicobacter pylori and early gastric cancer.Method A total of 82 people screened for gastric cancer were divided into early cancer group(n=20)and benign group(n=62)according to the results of gastroscopy and pathological examination.The clinical characteristics of the two groups were compared.The influenc-ing factors of occurrence of early gastric cancer were analyzed by Logistic regression analysis.The diagnostic value of PGⅡ,G-17,PGI,PGR single and combined detection for early gastric cancer was analyzed by receiver operating charac-teristic(ROC)curve and area under the curve(AUC).Result The age,helicobacter pylori antibody,family history of gastric cancer,smoking history,PGⅠ,PGⅡ,PGR,and G-17 of the two groups were compared,and the differences were statistically significant(P＜0.05).Logistic regression analysis showed that age,helicobacter pylori antibody,family histo-ry of gastric cancer,smoking history,PGⅠ,PGⅡ,PGR and G-17 were influencing factors of the occurrence of early gas-tric cancer(P＜0.05).ROC curve showed that the AUC of PGⅠ,PGⅡ,PGR,and G-17 combined detection in diagnosis of early gastric cancer were higher than those of each indicators single detection.Conclusion Age,helicobacter pylori anti-body,family history of gastric cancer,smoking history,PGⅠ,PGⅡ,PGR,G-17 were the influencing factors of the occur-rence of early gastric cancer.The combined detection of PGⅠ,PGⅡ,PGR and G-17 has high diagnostic value for early gas-tric cancer.
