Comparison of the effects of plasma radiofrequency ablation and CO2 laser in the treatment of early glottic carcinoma
Objective To compare the effects of plasma radiofrequency ablation and CO2 laser in the treament of ear-ly glottic carcinoma(EGC).Method A total of 80 EGC patients were divided into the control group(n=40,CO2 laser)and the study group(n=40,plasma radiofrequency ablation)according to the different treatment methods. The periopera-tive period indexes[operation time, intraoperative blood loss, postoperative hospital stay, and 24 h visual analogue scale(VAS)score after surgery],voice function[shimmer, harmonic to noise ratio(HNR),jitter],quality of life[voice handicap index(VHI)],swallowing function[video fluoroscopic swallowing study(VFSS)],incidence of local recurrence and com-plications of the two groups were compared.Result The operation time, postoperative hospital stay in the study group were significantly shorter than those in the control group, intraoperative blood loss was significantly less than that in the control group, and 24 h VAS score after surgery was significantly lower than that in the control group, and the differences were statistically significant(P<0.01).Four weeks after surgery, the HNR in the two groups were higher than those before surgery, jitter and shimmer were lower than those before surgery, the HNR in the study group was higher than that in the control group, jitter and shimmer were lower than those in the control group, and the differences were statistically signifi-cant(P<0.05).Six months after surgery, the VHI scores in the two groups were lower than those before surgery, the VFSS scores in the two groups were higher than those before surgery, the VHI score in the study group was lower than that in the control group,VFSS score in the study group was higher than that in the control group, and the differences were statis-tically significant(P<0.05).The local recurrence rate and total incidence of complications in the study group were lower than those in the control group, but the differences were not statistically significant(P>0.05).Conclusion Compared with CO2 laser, plasma radiofrequency ablation is more effective in the treatment of EGC,which can shorten the opera-tion time, reduce the intraoperative blood loss, and alleviate postoperative pain.At the same time,it has less impact on the voice and swallowing function, which can promote the rapid postoperative recovery and improve the quality of life.
