负荷剂量左甲状腺素钠对甲状腺乳头状癌术后患者的治疗效果

摘要
目的 探讨负荷剂量左甲状腺素钠对甲状腺乳头状癌(PTC)术后患者的治疗效果.方法 根据术后左甲状腺素钠应用剂量的不同将80例PTC患者分为对照组(n=40,普通剂量左甲状腺素钠)和研究组(n=40,负荷剂量左甲状腺素钠).比较两组患者的甲状腺功能指标[促甲状腺激素(TSH)、游离甲状腺素(FT4)、游离三碘甲腺原氨酸(FT3)和甲状腺过氧化物酶自身抗体(TPOAb)]、血管内皮生长因子(VEGF)水平、可溶性白细胞介素-2受体(sIL-2R)水平、细胞表面分化抗原44变异体6(CD44v6)水平及生活质量[欧洲癌症研究与治疗组织生命质量测定量表(EORTC QLQ-C30)].结果 治疗后,两组患者TSH、TPOAb水平均明显低于本组治疗前,FT4、FT3水平均明显高于本组治疗前,研究组患者TSH、TPOAb水平均明显低于对照组,FT4、FT3水平均明显高于对照组,差异均有统计学意义(P﹤0.01).治疗后,两组患者VEGF、sIL-2R、CD44v6水平均明显低于本组治疗前,研究组患者VEGF、sIL-2R、CD44v6水平均明显低于对照组,差异均有统计学意义(P﹤0.01).治疗1、2、3个月后,两组患者EORTC QLQ-C30评分均明显高于本组治疗前,研究组患者EORTC QLQ-C30评分均明显高于对照组,差异均有统计学意义(P﹤0.01).结论 负荷剂量左甲状腺素钠可改善PTC术后患者的甲状腺功能,降低VEGF、sIL-2R、CD44v6水平,改善患者的生活质量.
Abstract
Objective To investigate the therapeutic effect of loading dose levothyroxine sodium on patients with pap-illary thyroid carcinoma(PTC)after surgery.Method A total of 80 PTC patients were divided into control group(n=40,normal dose levothyroxine sodium)and study group(n=40,loading dose levothyroxine sodium)according to the differ-ence of postoperative levothyroxine sodium dose.The thyroid function indexes[thyroid stimulating hormone(TSH),free thyroxine(FT4),free triiodothyronine(FT3)and thyroid peroxidase autoantibody(TPOAb)],vascular endothelial growth factor(VEGF)level,soluble interleukin-2 receptor(sIL-2R)level,cluster of differentiation 44 variant 6(CD44v6)levels,and quality of life[European Organization for Research and Treatment of Cancer quality of life questionnaire core 30(EORTC QLQ-C30)]were compared between the two groups.Result After treatment,the TSH and TPOAb levels in the two groups were significantly lower than those before treatment,the FT4 and FT3 levels were significantly higher than those before treatment,the TSH and TPOAb levels in study group were significantly lower than those in control group,and the FT4 and FT3 levels were significantly higher than those in control group,and the differences were statistically sig-nificant(P<0.01).After treatment,the levels of VEGF,sIL-2R and CD44v6 in the two groups were significantly lower than those before treatment,and the levels of VEGF,sIL-2R and CD44v6 in the study group were significantly lower than those in the control group,and the differences were statistically significant(P<0.01).After 1,2 and 3 months of treat-ment,the EORTC QLQ-C30 scores of the two groups were significantly higher than those before treatment,and the EORTC QLQ-C30 scores of the study group were significantly higher than those of the control group,and the differences were statistically significant(P<0.01).Conclusion Loading dose levothyroxine sodium can improve the thyroid func-tion of PTC patients after surgey,reduce the levels of VEGF,sIL-2R,CD44v6 and improve the quality of life of patients.
