聚乙烯醇颗粒经导管动脉化疗栓塞术治疗晚期肝癌合并上消化道出血患者的疗效

摘要
目的 探讨聚乙烯醇颗粒经导管动脉化疗栓塞术治疗晚期肝癌合并上消化道出血患者的疗效.方法 根据治疗方式的不同将60例晚期肝癌合并上消化道出血患者分为对照组(n=30)和观察组(n=30),对照组患者给予经导管动脉化疗栓塞术,观察组患者在对照组的基础上给予聚乙烯醇颗粒栓塞治疗.比较两组患者的临床疗效、止血情况、肝功能指标[丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)、总胆红素(TBIL)]及预后情况.结果 观察组患者的治疗总有效率和止血率均高于对照组,3年复发率低于对照组,差异均有统计学意义(P﹤0.05).治疗后,两组患者TBIL、ALT、AST水平均低于本组治疗前,观察组患者TBIL、ALT、AST水平均低于对照组,差异均有统计学意义(P﹤0.05).结论 聚乙烯醇颗粒经导管动脉化疗栓塞术治疗晚期肝癌合并上消化道出血患者具有良好的临床疗效和止血效果,可以显著改善患者的肝功能指标,并且降低复发率.
Abstract
Objective To investigate the efficacy of polyvinyl alcohol granules transcatheter arterial chemoemboliza-tion in the treatment of advanced liver cancer patients complicated with upper gastrointestinal bleeding.Method A total of 60 patients with advanced liver cancer complicated with upper gastrointestinal bleeding were divided into control group(n=30)and observation group(n=30)according to different treatment methods.The control group was treated with transcatheter arterial chemoembolization,and the observation group was treated with polyvinyl alcohol granules emboli-zation on the basis of the control group.The clinical efficacy,hemostasis,liver function indexes[alanine aminotransferase(ALT),aspartate aminotransferase(AST),total bilirubin(TBIL)]and prognosis of the two groups were compared.Result The total effective rate and hemostasis rate in observation group were higher than those in control group,the 3-year recur-rence rate was lower than that in control group,and the differences were statistically significant(P<0.05).After treat-ment,the levels of TBIL,ALT and AST in two groups were lower than those before treatment,and the levels of TBIL,ALT and AST in observation group were lower than those in control group,and the differences were statistically signifi-cant(P<0.05).Conclusion The polyvinyl alcohol granules transcatheter arterial chemoembolization in the treatment of advanced liver cancer patients complicated with upper gastrointestinal bleeding has good clinical efficacy and hemostatic effect,which can significantly improve the liver function indexes of patients and reduce the recurrence rate.
