Diagnostic value of dynamic contrast enhanced-magnetic resonance imaging combined with diffusion weighted imaging in brain glioma

摘要
目的 探讨动态对比增强磁共振成像(DCE-MRI)联合弥散加权成像(DWI)对脑胶质瘤的诊断价值.方法 收集86例脑胶质瘤患者的临床资料,所有患者均进行MRI平扫、DWI、DCE-MRI检查.根据组织学分级的不同将脑胶质瘤患者分为低级别组(n=39,Ⅰ~Ⅱ级)和高级别组(n=47,Ⅲ~Ⅳ级),比较两组患者的DCE-MRI、DWI扫描参数.绘制受试者工作特征(ROC)曲线,计算曲线下面积(AUC),分析DCE-MRI、DWI单独及联合检查对脑胶质瘤的诊断价值.结果 低级别组患者的表观弥散系数(ADC)明显高于高级别组,容量转移常数(Ktrans)、血管外细胞外间隙容积分数(Ve)均明显低于高级别组,差异均有统计学意义(P﹤0.01).两组患者的速率常数(Kep)、血浆容积分数(Vp)比较,差异均无统计学意义(P﹥0.05).ROC曲线显示,DCE-MRI和DWI联合检查诊断脑胶质瘤的AUC为0.979(95%CI:0.752~0.986),灵敏度为95.06%,特异度为87.25%,均高于DCE-MRI、DWI单独检查.结论 DCE-MRI参数和ADC可为脑胶质瘤的分级提供依据,DCE-MRI和DWI联合检查对脑胶质瘤具有较高的诊断价值.
Abstract
Objective To explore the diagnostic value of dynamic contrast enhanced-magnetic resonance imaging(DCE-MRI)combined with diffusion weighted imaging(DWI)in brain glioma.Method The clinical data of 86 patients with brain glioma were collected.All patients underwent MRI plain scan,DWI and DCE-MRI examination.The patients with brain glioma were divided into low-grade group(n=39,grade Ⅰ-Ⅱ)and high-grade group(n=47,grade Ⅲ-Ⅳ)accord-ing to different histological grades.The DCE-MRI and DWI scanning parameters of the two groups were compared.The receiver operating characteristic(ROC)curve was plotted,the area under the curve(AUC)was calculated,and the diag-nostic value of DCE-MRI,DWI single and combined examination in brain glioma was analyzed.Result The apparent diffusion coefficient(ADC)in low-grade group was significantly higher than that in high-grade group,and the volume transfer constant(Ktrans)and extravascular extracellular volume fraction(Ve)in low-grade group were significantly lower than those in high-grade group,and the differences were statistically significant(P<0.01).There were no significant dif-ferences in rate constant(Kep)and fractional plasma volume(Vp)between the two groups(P>0.05).ROC curve showed that the AUC of DCE-MRI and DWI combined examination for the diagnosis of brain glioma was 0.979(95%CI:0.752-0.986),the sensitivity was 95.06%,and the specificity was 87.25%,which were higher than those of DCE-MRI and DWI single examination.Conclusion DCE-MRI parameters and ADC can provide a basis for the classification of brain glio-ma,and the combined examination of DCE-MRI and DWI has high diagnostic value for brain glioma.
