Comparison of the effect of emergency surgery and elective surgery after stent dilation in the treatment of right colon cancer patients complicated with acute intestinal obstruction
Objective To compare the effect of emergency surgery and elective surgery after stent dilation in the treat-ment of right colon cancer patients complicated with acute intestinal obstruction.Method A total of 96 right colon can-cer patients complicated with acute intestinal obstruction were divided into control group(n=46,emergency surgery)and observation group(n=50,elective surgery after stent dilation)according to different treatment methods.The perioperative period indexes,gastrointestinal hormone indexes[motilin(MTL),gastrin(GAS),vasoactive intestinal peptide(VIP)],in-flammatory factor indexes[interleukin(IL)-6,IL-8,tumor necrosis factor-α(TNF-α)],incidence of complications after surgery and survival were compared between the two groups.Result The amount of intraoperative blood loss in observa-tion group was significantly less than that in control group,the first time of anal exhaust,recovery time of liquid diet and postoperative hospital stay were significantly shorter than those in control group,the number of lymph node dissection was significantly higher than that in control group,and the differences were statistically significant(P<0.05).After sur-gery,the levels of MTL,GAS,VIP,IL-6,IL-8 and TNF-α in two groups were lower than those before surgery,the levels of MTL,GAS and VIP in observation group were higher than those in control group,the levels of IL-6,IL-8 and TNF-α were lower than those in control group,and the differences were statistically significant(P<0.05).The total incidence of postoperative complications in observation group was lower than that in control group,and the 3-year survival rate was higher than that in control group,and the differences were statistically significant(P<0.05).Conclusion Compared with emergency surgery,elective surgery after stent dilation can reduce intraoperative blood loss,increase the number of lymph node dissection,promote postoperative recovery,reduce the levels of gastrointestinal hormones and inflammatory factors,reduce postoperative complications,and improve postoperative survival rate.
