Evaluation value of ultrasound blood flow parameter,serum preablative stimulated thyroglobulin/thyroid stimulating hormone combined detection for postoperative recurrence of thyroid cancer in children
Objective To investigate the evaluation value of ultrasound blood flow parameters,serum preablative stimulated thyroglobulin(ps-Tg)/thyroid stimulating hormone(TSH)combined detection for postoperative recurrence of thyroid cancer in children.Method A total of 42 children with thyroid cancer were selected,all of whom underwent sur-gical treatment.Based on postoperative recurrence,they were divided into recurrent group and non-recurrent group.The blood flow grading,ultrasound blood flow parameters[resistance index(RI),pulsatility index(PI),systolic peak velocity/diastolic end velocity(S/D)],serological indicators[thyroglobulin(Tg),TSH,ps-Tg,thyroglobulin antibody(TgAb),ps-Tg/TSH]were compared between two groups.Using pathological examination results as the gold standard,the evaluation value of ultrasound blood flow parameters,ps-Tg/TSH single and combined detection for postoperative recurrence in chil-dren with thyroid cancer were evaluated.Kappa test was used for consistency analysis.Result Among the 42 postopera-tive children with thyroid cancer,24 had recurrence and 18 had no recurrence.There was no statistically significant differ-ence in blood flow grading between the recurrent group and the non-recurrent group(P>0.05).The RI,PI,S/D,ps-Tg,and ps-Tg/TSH of the recurrent group were significantly higher than those of the non-recurrent group,and the differences were statistically significant(P<0.01).There were no statistically significant differences in the levels of TgAb and TSH between the two groups(P>0.05).The sensitivity,specificity,and accuracy of ultrasound blood flow parameters,ps-Tg/TSH combined detection in diagnosing postoperative recurrence in children with thyroid cancer were 91.67%,88.89%,and 90.48%,respectively.The consistency between ultrasound blood flow parameters,ps-Tg/TSH combined dection in di-agnosing postoperative recurrence in children with thyroid cancer and pathological examination was high(Kappa=0.806),and the combined detection had a better evaluation value for postoperative recurrence in children with thyroid cancer than the single detection.Conclusion The ultrasound blood flow parameters,ps-Tg/TSH combined detection has high evalua-tion value for postoperative recurrence of thyroid cancer in children,and its diagnostic sensitivity,specificity,and accura-cy are higher than those of the single detection.
