精准营养干预对前列腺癌术后化疗患者营养状态、化疗不良反应的影响

Effects of precision nutrition intervention on nutritional status and adverse reactions of prostate cancer patients undergoing postoperative chemotherapy

目的 探讨精准营养干预对前列腺癌(PCa)术后化疗患者营养状态、化疗不良反应的影响.方法 将90例PCa术后化疗患者根据干预方案的不同分为对照组47例和营养组43例.对照组采取常规干预,营养组采取精准营养干预.比较两组患者主观营养状态[患者主观整体营养评估量表(PG-SGA)]、客观营养状态[转铁蛋白(TRF)、白蛋白(ALB)、前白蛋白(PAB)]、化疗不良反应发生情况以及生活质量[欧洲癌症研究与治疗组织生命质量测定量表(EORTC QLQ-C30)].结果 干预后,营养组患者主观营养状态明显优于对照组,差异有统计学意义(P<0.01).干预后,两组患者ALB、PA、TRF均较干预前上升,且营养组患者ALB、PAB、TRF均高于对照组,差异均有统计学意义(P<0.05).干预后,两组患者EORTC QLQ-C30各维度评分均较干预前上升,且营养组患者EORTC QLQ-C30各维度评分均高于对照组,差异均有统计学意义(P<0.05).营养组患者化疗不良反应总发生率低于对照组,差异有统计学意义(P<0.05).结论 精准营养干预能有效提高PCa根治术后化疗患者的营养状态,并能减轻其化疗不良反应,对提高患者的生活质量具有一定的优势.
Objective To investigate the effects of precision nutrition intervention on nutritional status and adverse re-actions of prostate cancer(PCa)patients undergoing postoperative chemotherapy.Method A total of 90 postoperative chemotherapy patients with PCa were divided into control group(47 cases)and nutrition group(43 cases)according to different intervention programs.The control group received routine intervention,and the nutrition group received preci-sion nutrition intervention.The subjective nutritional status[patient generated subjective global assessment(PG-SGA)],objective nutritional status[transferrin(TRF),albumin(ALB),prealbumin(PAB)],incidence of chemotherapy adverse re-actions and quality of life[European Organization for Research and Treatment of Cancer quality of life questionnaire core 30(EORTC QLQ-C30)]were compared between the two groups.Result After intervention,the objective nutritional sta-tus of the nutritional group was significantly better than that of the control group,the difference was statistically signifi-cant(P<0.01).After intervention,ALB,PAB and TRF in two groups were higher than those before intervention,and ALB,PAB and TRF in nutrition group were higher than those in control group,the differences were statistically signifi-cant(P<0.05).After intervention,the EORTC QLQ-C30 scores in each dimension in both groups were higher than those before intervention,and the EORTC QLQ-C30 scores in each dimension in nutrition group were higher than those in con-trol group,the differences were statistically significant(P<0.05).The total incidence of chemotherapy adverse reactions in nutrition group was lower than that in control group,and the difference was statistically significant(P<0.05).Conclusion Precision nutrition intervention can effectively improve the nutritional status of patients with chemotherapy after PCa sur-gery,and reduce the adverse reactions of chemotherapy,which has certain advantages in improving the quality of life.
