丙型病毒性肝炎肝癌患者丙型肝炎病毒-RNA载量与甲胎蛋白、癌胚抗原的关系

摘要
目的 探讨丙型病毒性肝炎肝癌患者丙型肝炎病毒(HCV)-RNA载量与甲胎蛋白(AFP)、癌胚抗原(CEA)的关系.方法 选取37例丙型病毒性肝炎肝癌患者,使用全自动化学发光测定仪检测血清CEA、AFP水平,采用荧光定量聚合酶链反应法检测HCV-RNA载量,根据HCV-RNA载量检测结果分为HCV高载量组(n=20)和HCV低载量组(n=17),比较两组患者的CEA、AFP水平.采用Spearman相关性分析法分析HCV-RNA载量与CEA、AFP的相关性.采用受试者工作特征(ROC)曲线及曲线下面积(AUC)分析CEA、AFP单独及联合检测对HCV-RNA高载量丙型病毒性肝炎肝癌的诊断价值.结果 HCV-RNA高载量组患者CEA、AFP水平均高于HCV-RNA低载量组,差异均有统计学意义(P﹤0.05).HCV-RNA载量与CEA、AFP水平均呈正相关(r=0.321、0.284,P﹤0.05).ROC曲线显示,CEA和AFP联合检测诊断HCV-RNA高载量丙型病毒性肝炎肝癌的AUC最大.结论 HCV-RNA高载量丙型病毒性肝炎肝癌患者的CEA、AFP水平较高,且HCV-RNA载量与CEA、AFP水平呈正相关.CEA和AFP联合检测对HCV-RNA高载量丙型病毒性肝炎肝癌具有一定的诊断价值.
Abstract
Objective To investigate the relationship between hepatitis C virus(HCV)-RNA load and α-fetoprotein(AFP),carcinoembryonic antigen(CEA)in patients with viral hepatitis C liver cancer.Method A total of 37 patients with viral hepatitis C liver cancer were selected,the serum CEA and AFP levels were detected by automatic chemilumi-nescence analyzer,the HCV-RNA load was detected by fluorescence quantitative polymerase chain reaction method.All patients were divided into HCV-RNA high load group(n=20)and HCV-RNA low load group(n=17)according to the HCV-RNA load detection results,and the CEA and AFP levels of the two groups were compared.The correlation be-tween HCV-RNA load and CEA,AFP was analyzed by Spearman correlation analysis.The diagnostic value of CEA and AFP single and combined detection for viral hepatitis C liver cancer patients with HCV-RNA high load was analyzed by receiver operating characteristic(ROC)and area under the curve(AUC).Result The CEA and AFP levels in the HCV-RNA high load group were higher than those in the HCV-RNA low load group,and the differences were statistically sig-nificant(P<0.05).HCV-RNA load was positively correlated with CEA and AFP levels(r=0.321,0.284,P<0.05).ROC curve showed that the AUC of CEA and AFP combined detection in the diagnosis of viral hepatitis C liver cancer with HCV-RNA high load was the largest.Conclusion The CEA and AFP levels are high in viral hepatitis C liver cancer pa-tients with HCV-RNA high load,and the HCV-RNA load is positively correlated with CEA and AFP levels.The com-bined detection of CEA and AFP has a certain value in the diagnosis of viral hepatitis C liver cancer with HCV-RNA high load.
