摘要

目的 探讨彩色多普勒血流成像(CDFI)血流参数及图像特征对分化型甲状腺癌(DTC)伴转移患者术后131I清灶治疗疗效的预测价值.方法 选取120例DTC伴转移患者,术后均接受131I清灶治疗.依据131I清灶治疗疗效分为疗效满意(ER)组(n=75)和效果不佳组(n=45).绘制受试者工作特征(ROC)曲线,计算曲线下面积(AUC),评估CDFI血流参数及图像特征单独及联合检测对DTC伴转移患者术后131I清灶治疗疗效的预测效能.结果 ER组与效果不佳组患者颈侧区淋巴结转移、中央区淋巴结转移、服碘剂量、收缩期峰值流速/舒张末期流速(S/D)、阻力指数(RI)、纵横比、血流情况比较,差异均有统计学意义(P＜0.05). ROC曲线显示,S/D、RI、血流、纵横比联合检测预测DTC伴转移患者术后131I清灶治疗疗效的AUC为0.954(95％CI:0.920～0.988),高于各指标单独检测,此时的灵敏度为0.800,特异度为0.987.结论 CDFI血流参数及图像特征可有效预测DTC伴转移患者术后131I清灶治疗效果,以指导临床治疗.

Abstract

Objective To investigate the predictive value of color Doppler flow imaging(CDFI)blood flow parame-ters and image features for 131I ablation treatment efficacy in patients with differentiated thyroid cancer(DTC)with metas-tasis.Method A total of 120 DTC with metastasis patients were selected,all of whom received 131I ablation treatment af-ter surgery.According to the therapeutic effect of 131I ablation treatment,the patients were divided into excellent response(ER)group(n=75)and ineffective group(n=45).The receiver operating characteristic(ROC)curve was drawn,and the ar-ea under the curve(AUC)was evaluated,the predictive value of CDFI blood flow parameters and image features single and combined detection for postoperative 131I ablation treatment efficay in DTC patients with metastasis were evaluated.Result There were significant differences in cervical lymph node metastasis,central lymph node metastasis,iodine dos-age,peak systolic velocity/end diastolic velocity(S/D),resistance index(RI),aspect ratio,and blood flow between the ER group and the ineffective group(P＜0.05).The ROC curve showed that the AUC of the S/D,RI,blood flow,and aspect ra-tio combined detection for predicting the efficacy of 131I ablation treatment in DTC patients with metastasis after surgery was 0.954(95％CI:0.920-0.988),which was higher than the single detection of each indicator.At this time,the sensitivity was 0.800,and the specificity was 0.987.Conclusion CDFI blood flow parameters and image features can effectively predict postoperative 131I ablation treatment effect in DTC patients with metastasis to guide clinical treatment.
