目的 探讨剪切波弹性成像(SWE)、超声引导下穿刺活检联合检查对甲状腺良恶性肿瘤的鉴别诊断价值.方法 选取57例甲状腺肿瘤患者,均接受超声引导下穿刺活检和SWE检查,以手术病理检查结果为金标准,评估SWE、超声引导下穿刺活检单独及联合检查对甲状腺良恶性肿瘤的鉴别诊断价值.SWE、超声引导下穿刺活检单独及联合检查鉴别诊断甲状腺良恶性肿瘤的结果与病理检查结果的一致性采用Kappa检验.结果 术后病理检查结果显示,57例甲状腺肿瘤患者中,良性20例,恶性37例.SWE检查诊断甲状腺恶性肿瘤的灵敏度为67.57%,特异度为90.00%,准确度为75.44%,阳性预测值为92.59%,阴性预测值为60.00%,其与病理检查结果的一致性中等(Kappa=0.516).超声引导下穿刺活检诊断甲状腺恶性肿瘤的灵敏度为72.97%,特异度为85.00%,准确度为77.19%,阳性预测值为90.00%,阴性预测值为62.96%,其与病理检查结果的一致性中等(Kap-pa=0.537).SWE联合超声引导下穿刺活检诊断甲状腺恶性肿瘤的灵敏度为86.49%,特异度为80.00%,准确度为84.21%,阳性预测值为88.89%,阴性预测值为76.19%,其与病理检查结果的一致性较高(Kappa=0.657);SWE联合超声引导下穿刺活检诊断甲状腺恶性肿瘤的灵敏度、准确度及阴性预测值均高于二者单独检查.结论 SWE、超声引导下穿刺活检联合检查鉴别诊断甲状腺良恶性肿瘤的价值较高,与病理检查结果的一致性较高.
Objective To explore the differential diagnostic value of shear wave elastography(SWE),ultrasound-guided biopsy combined examination for benign and malignant thyroid tumors.Method A total of 57 patients with thy-roid tumors were selected,all of whom underwent ultrasound-guided biopsy and SWE examination.The results of surgi-cal pathological examination was used as the gold standard,and the differential diagnostic value of SWE,ultrasound-guid-ed biopsy single and combined examinations for benign and malignant thyroid tumors were evaluated.The consistency between the results of SWE,ultrasound-guided biopsy single and combined examinations for differential diagnosis of be-nign and malignant thyroid tumors and pathological examination results was determined by Kappa test.Result The post-operative pathological examination results showed that among the 57 patients with thyroid tumors,20 cases were benign and 37 cases were malignant.The sensitivity,specificity,accuracy,positive predictive value,and negative predictive val-ue of SWE single examination in diagnosing thyroid malignant tumors were 67.57%,90.00%,75.44%,92.59%,and 60.00%,respectively,and the consistency with pathological examination results was moderate(Kappa=0.516).The sensi-tivity,specificity,accuracy,positive predictive value,and negative predictive value of ultrasound-guided biopsy single ex-amination in diagnosing thyroid malignant tumors were 72.97%,85.00%,77.19%,90.00%,and 62.96%,respectively,and the consistency with pathological examination results was moderate(Kappa=0.537).The sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of SWE and ultrasound-guided biopsy combined examination in diagnosing thyroid malignant tumors were 86.49%,80.00%,84.21%,88.89%,and 76.19%,respectively,and the consis-tency with pathological examination results was high(Kappa=0.657).The sensitivity,accuracy and negative predictive value of SWE,ultrasound-guided biopsy combined examination in diagnosing thyroid malignant tumors were higher than those of the single examination.Conclusion SWE,ultrasound-guided biopsy combined examination has a high value in the differential diagnosis of benign and malignant thyroid tumors,and the consistency with pathological examination re-sults is high.
