摘要
目的 探讨通关藤注射液对晚期胃癌化疗患者免疫功能及炎症因子的影响.方法 根据治疗方法的不同将84例晚期胃癌患者分为对照组[n=44,5-氟尿嘧啶+亚叶酸钙+奥沙利铂(FOLFOX6)方案治疗]和观察组(n=40,FOLFOX6方案+通关藤注射液治疗).比较两组患者的临床疗效、血清肿瘤标志物[癌胚抗原(CEA)、糖类抗原125(CA125)、糖类抗原19-9(CA19-9)]水平、免疫功能指标(CD4+、CD8+、CD4+/CD8+)、炎症因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、超敏C反应蛋白(hs-CRP)]、生活质量[生活质量调查问卷-C30(QLQ-C30)]及不良反应发生情况.结果 观察组患者客观缓解率和临床获益率均高于对照组,差异均有统计学意义(P﹤0.05).治疗后,两组患者CEA、CA125、CA19-9、TNF-α、IL-6、hs-CRP水平均较治疗前降低,且观察组患者CEA、CA125、CA19-9、TNF-α、IL-6、hs-CRP水平均低于对照组,差异均有统计学意义(P﹤0.05).治疗后,两组患者CD4+水平及CD4+/CD8+均较治疗前升高,且观察组患者CD4+水平及CD4+/CD8+均高于对照组,差异均有统计学意义(P﹤0.05).治疗后,两组患者功能领域、症状领域、单一条目评分均较治疗前降低,总健康水平评分均较治疗前升高,且观察组患者功能领域、症状领域、单一条目评分均低于对照组,总健康水平评分高于对照组,差异均有统计学意义(P﹤0.05).观察组患者各不良反应发生率均低于对照组,差异均有统计学意义(P﹤0.05).结论 晚期胃癌患者在化疗时加用通关藤注射液可以降低肿瘤标志物、炎症因子水平,提高免疫功能和生活质量,减少不良反应,临床疗效较好.
Abstract
Objective To investigate the effects of Tongguanteng injection on immune function and inflammatory fac-tors in patients with advanced gastric cancer undergoing chemotherapy.Method According to different treatment meth-ods,84 patients with advanced gastric cancer were divided into control group[n=44,5-fluorouracil+calcium folinate+ox-aliplatin(FOLFOX6)regimen]and observation group(n=40,FOLFOX6 regimen+Tongguanteng injection).Clinical efficacy,serum tumor markers[carcinoembryonic antigen(CEA),carbohydrate antigen 125(CA125),carbohydrate an-tigen 19-9(CA19-9)],immune function indicators(CD4+,CD8+,CD4+/CD8+),inflammatory factors[tumor necrosis fac-tor-α(TNF-α),interleukin-6(IL-6),hypersensitive C-reactive protein(hs-CRP)],quality of life[quality of life question-naire-C30(QLQ-C30)]and adverse reactions were compared between the two groups.Result The objective response rate and clinical benefit rate of the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).After treatment,the levels of CEA,CA125,CA19-9,TNF-α,IL-6 and hs-CRP in two groups were lower than those before treatment,and the levels of CEA,CA125,CA19-9,TNF-α,IL-6 and hs-CRP in observation group were lower than those in control group,the differences were statistically significant(P<0.05).After treatment,the level of CD4+and CD4+/CD8+in two groups were higher than those before treatment,and the levels of CD4+and CD4+/CD8+in observation group were higher than those in control group,the differences were statistically sig-nificant(P<0.05).After treatment,the scores of functional area,symptom area and single item in both groups were lower than those before treatment,and the scores of total health level were higher than those before treatment,the scores of func-tional area,symptom area and single item in observation group were lower than those in control group,and the score of to-tal health level was higher than that in control group,the differences were statistically significant(P<0.05).The incidenc-es of various adverse reactions in observation group were lower than those in control group,the differences were statisti-cally significant(P<0.05).Conclusion In patients with advanced gastric cancer undergoing chemotherapy,Tongguan-teng injection can reduce the levels of tumor markers and inflammatory factors,improve the immune function and quali-ty of life,reduce adverse reactions,and the clinical efficacy is good.
