Application effect of fentanyl combined with propofol for sedation and analgesia in children with leukemia undergoing bone marrow puncture or intrathecal injection
Objective To investigate the application effect of fentanyl combined with propofol for sedation and anal-gesia in children with leukemia undergoing bone marrow puncture or intrathecal injection.Method A total of 96 chil-dren with leukemia were selected, and all of them received bone marrow puncture or intrathecal injection, they were di-vided into observation group(n=45)and control group(n=51)according to different sedation and analgesia regimens. The control group received lidocaine combined with propofol for sedation and analgesia, while the observation group re-ceived fentanyl combined with propofol for sedation and analgesia. The perioperative indicators, hemodynamic indicators[heart rate, mean arterial pressure(MAP)],awakening quality[modified Aldrete scoring system(MASS)],pain level[face pain scale-revision(PFS-R)],and incidence of complications were compared between the two groups.Result The surgical operation time and postoperative awakening time in observation group were significantly shorter than those in control group, and the intraoperative propofol dosage was significantly less than that in control group, and the differences were statistically significant(P<0.01).Immediately after bone marrow puncture or intrathecal injection(T1),the heart rate and MAP in two groups were lower than those before the surgery begin(T0),and the MAP in observation group was high-er than that in control group, and the differences were statistically significant(P<0.05).At 5,30 min after surgery, the MASS scores in observation group were significantly higher than those in control group, and the differences were statisti-cally significant(P<0.01).At 60 min after surgery, the FPS-R score in observation group was significantly lower than that in control group, and the difference was statistically significant(P<0.01).The total incidence of complications in the ob-servation group was 13.33%,which was lower than 33.33%in the control group, and the difference was statistically sig-nificant(P<0.05).Conclusion Fentanyl combined with propofol has significant sedative and analgesic effects in chil-dren with leukemia undergoing bone marrow puncture and intrathecal injection. It can stabilize the hemodynamic indica-tors of children, shorten the operation time and recovery time, improve the quality of recovery, and has good safety.
