Objective:To investigate the status of discharge readiness of stroke patients and their caregivers,analyze the possible influencing factors,and provide the basis for discharge planning services. Methods:A total of 130 stroke patients and their caregivers were investigated with readiness for hospital discharge scale and caregiver readiness scale before discharge. Results:There was a positive correlation between patients' discharge readiness and caregivers' readiness (P<0. 01),and a positive correlation between coping capacity and caregiver readiness (P<0. 01). According to the results of multiple regression analysis,the total scores of patients' readiness were significantly affected by the following factors:the main source of the costs for hospitalization,family monthly income,gender and marital status of caregivers and caregiving experience of caregivers (P<0. 01),while another four factors,the gender of patients,family monthly income,education level and response capacity of caregivers,had strong influence on the total scores of caregivers' readiness (P<0. 01). Conclusions:The readiness of stroke patients and caregivers needs to be improved. It should be assessed in a timely manner of readiness levels of patients and caregivers,and the personalized discharge guidance program should be given to realize the effective discharge transition.
