摘要
目的:探讨血清中糖类抗原CA125、CA199、CA724以及癌胚抗原(CEA)单一或联合检测在胃恶性肿瘤腹膜转移诊断中的临床应用价值.方法:选择胃癌腹膜转移组病人68例,胃癌无腹膜转移组病人44例,另选择在医院体检中心进行健康体检的40名体检者作为对照组.记录并分析病人血清CA125、CA199、CA724、CEA表达的变化情况,分析血清CA125、CA199、CA724以及CEA在诊断胃恶性肿瘤腹膜转移中的效能.结果:对照组血清CA125、CA199、CA724、CEA水平低于腹膜转移组和无腹膜转移组,无腹膜转移组各因子水平低于腹膜转移组,差异均有统计学意义(P<0.05～P<0.01),腹膜转移组四项联合阳性率95.83％,无腹膜转移组四项联合阳性率为86.36％,2组差异有统计学意义(P<0.05).单项指标检测中,CA125、CA199、CA724、CEA所对应的AUC分别为0.925、0.655、0.782、0.801,其中CA125的值最高,诊断效能最高,血清中的CA125指标的灵敏度和特异度亦高.结论:血清CA125、CA199、CA724以及CEA四项联合检测对于胃恶性肿瘤腹膜转移诊断及鉴别诊断具有重要的价值.
Abstract
Objective:To investigate the clinical value of single or combined detection of serum carbohydrate antigens CA125,CA199,CA724 and carcinoembryonic antigen(CEA) in the diagnosis of peritoneal metastasis of gastric malignant tumors.Methods:The gastric cancer peritoneal metastasis group (68 cases),gastric cancer non-peritoneal metastasis group (44 cases) and healthy physical examination(control group,40 cases) were investigated.The serum levels of CA125,CA199,CA724 and CEA were recorded,and the efficacy of serum CA125,CA199,CA724 and CEA in the diagnosis of peritoneal metastasis of gastric malignant tumors were analyzed.Results:The levels of serum CA125,CA199,CA724 and CEA in the control group were lower than those in peritoneal metastasis group and non-peritoneal metastasis group,while the levels of all factors in non-peritoneal metastasis group were lower than those in peritoneal metastasis group,and the differences were statistically significant(P<0.05 to P<0.01).The combined positive rates of four items in the peritoneal metastasis group and non-peritoneal metastasis group were 95.83％ and 86.36％,respectively,and the difference of which was statistically significant(P<0.05).In single index detection,the AUC corresponding to CA125,CA199,CA724 and CEA were 0.925,0.655,0.782 and 0.801,respectively.Among fours items,CA125 was the highest value and highest diagnostic efficacy.In addition,the sensitivity and specificity of serum CA125 were also high.Conclusions:The combined detection of serum CA125,CA199,CA724 and CEA is of great value in the diagnosis and differential diagnosis of peritoneal metastasis of gastric malignant tumors.
