Epidemiological Characteristics and Viral Genetic Analysis of Rubella in Xinjiang Uygur Autonomous Region,China,2018-2023
We wished to understand the epidemiological characteristics of rubella and genetic characteristics of the rubella virus(RuV)in Xinjiang Uygur Autonomous Region(hereafter termed"Xinjiang"),China, during 2018-2023.We collated and summarized the data on the incidence of rubella in Xinjiang,China during 2018-2023.We collected clinical samples in 2022 for nucleic acid detection and virus isolation, and sequenced the strains to obtain their target gene sequences(739 target fragments within the E1 gene,E1-739).Based on E1-739,a phylogenetic tree was constructed to identify the genotype/lineage of RuV strains in the present study with World Health Organization representative genotyping reference strains as well as the reported representative lineage reference strains of genotypes 1E and 2B.In addition, comparisons were made with representative strains of RuV prevalent in other provinces of China during the same period to understand the molecular differences of RuV prevalent in Xinjiang.A total of 599 cases of rubella were reported in Xinjiang,China during 2018-2023.Except for 2019,when the incidence of rubella had a slight rebound(414 cases,1.66/100,000),the incidence rate in the remaining years was maintained at a relatively low level(average rate:0.17/100,000),and was reduced to the lowest level in history in 2023(eight cases,0.03/100,000).During 2018-2023,cases were concentrated mainly in northern Xinjiang,China(Altay and Tacheng). The proportion of cases among children aged<10 years was high in 2018 and 2021-2023(52.8%-62.5%),whereas in 2019 and 2020,the age group of 10-19 years was predominant(72.4%and 51.9%).Data for virological surveillance showed that seven RuV strains belonging to 1E-L2(six strains)and 2B-L2c(one strain)were obtained from four cities of Xinjiang,China in 2022,which had a high degree of homology with RuV strains prevalent in other Provinces of China during the same period. Our study provides basic scientific data for the development of effective prevention and control strategies for rubella in Xinjiang,China.
