摘要

目的探讨在组织病理学及血清特异性抗体检测基础上,联合直接免疫荧光技术(DIF)对黏膜类天疱疮(MMP)诊断效能的影响.方法纳入MMP患者 29 例,对照组 29 例,切取活检组织分别进行苏木精-伊红(HE)及DIF染色,对仅使用组织病理学及血清特异性抗体检测,和联合使用DIF后两种诊断方法进行诊断效能评价,分析多种因素对组织病理学及DIF诊断准确性的影响.结果联合使用DIF后可以提高MMP的诊断敏感度、阴性预测值、诊断符合率和受试者操作特征(ROC)曲线下面积.病程长短对DIF诊断准确性有显著影响,病程越长,DIF诊断准确性越高(P<0.05).结论 DIF联合组织病理学及血清抗体检测可以提高MMP的诊断准确性.对于疑难病例可能需要通过多次、多部位的活检提高DIF对MMP的诊断率.

Abstract

Objective To investigate the effects of DIF combined with histopathology and ELISA on the diagnostic accuracy of MMP.Methods Biopsy tissues were collected from patients with MMP(n=29)and without(n=29).Tissues were stained by DIF and HE respectively.To analysis the diagnostic efficacy of two diagnostic methods(using only histopathology and ELISA,and combining with DIF)and the impact of multiple factors on the accuracy of histopathology and DIF.Results The combination of DIF can improve the sensitivity,negative predictive value,diagnostic accuracy and area under curve of MMP.The course of disease has a significant impact on the diagnostic accuracy of DIF.The longer course,the higher accuracy(P<0.05).Conclusions DIF combined with histopathology and ELISA can improve the accuracy for the diagnosis of MMP.For difficult cases,multiple and multi-site biopsies may be necessary to improve the diagnostic accuracy of DIF on MMP.
