Numerical simulation of nasal irrigation at three different head positions
Objective To evaluate the effect of three different head positions on nasal irrigation. Methods In this paper, the nasal cavity model is constructed according to the CT scan images of the nasal cavity of healthy adults. The effects of three different head positions of anterior lateral position, lateral position and supine position are studied by Ansys Fluent 2022R2 software and computational fluid dynamics method. The real-time coverage rates of the flushing fluid in the whole nasal cavity and the upper part of the nasal cavity are obtained. Results In nasal irrigation, the flushing efficiency results of three different head positions are as follows:when the flushing time is within 0 to 0.1 s,the overall flushing fluid coverage of the three head positions is roughly the same.At the range of 0.1 s to 0.4 s,the coverage rate of flushing fluid in the lateral position is the highest, the supine position is the lowest. When the semi-rinse period is reached, the washing period of the supine position and the lateral position is the shortest, and the period of the anterior lateral position is slightly longer, but the coverage rate is the highest. When completing a flushing cycle, the lateral position has the shortest flushing cycle, while the anterior lateral position has the highest flushing fluid coverage rate.In the process of flushing the upper part of the nasal cavity, the coverage rate of the lateral position is the highest, and the flushing fluid is easier to enter the position of the sinus ostium, and the flushing effect is the best for sinus-related diseases.According to the results of the velocity streamline diagram,it is found that when the flushing fluid enters the nasal cavity in the lateral position, the flow rate is gentle.Only a small amount of water flows slowly into the nasopharynx, and the risk of choking water is not high. When the supine position is used, the flow rate of the flushing fluid increases sharply after entering the nasal cavity, and a large amount of flushing fluid enters the nasopharynx at a faster flow rate,so there is a risk of choking water. When the anterior lateral position is used, because of the influence of the flushing angle, the flow rate of the flushing fluid is accelerated after most of the flushing fluid enters the upper nasal meatus, and a faster flushing flow rate is always maintained in the narrow nasal meatus, which can effectively take away the mucus, allergens and other substances in the nasal cavity. Conclusions The lateral position has the highest coverage rate of the upper part of the nasal cavity, which is more suitable for the treatment of sinus-related diseases, and the flow velocity is slow, the flow into the nasopharynx is less, and the comfort is high.It is the first choice for children's nasal irrigation. The anterior and lateral position has good flushing effect, simple operation, and is suitable for adults with high medication compliance.
