老年高血压患者血清MCP-1、NLRP3炎性小体表达水平与肠道菌群的关系研究

目的:研究老年高血压患者血清单核细胞趋化蛋白-1(monocyte chemoattractant pro-tein-1,MCP-1)和NOD样受体蛋白3(NOD-like receptor protein 3,NLRP3)炎性小体表达与肠道菌群关系.方法:选择2019 年 1 月-2021 年 1 月本院收治的84 例老年原发性高血压患者,依据2018 年欧洲高血压指南将所有高血压患者分为极高危组(28 例)、高危组(38 例)和低中危组(18 例),对患者肠道菌群,以及血清MCP-1、NLRP3 炎性小体水平进行检测.结果:三组患者血压、血清肌酐以及临床并发症差异具有统计学意义(P<0.05),患者其余各指标差异均无统计学意义(P>0.05).极高危组的肠道双歧杆菌和乳杆菌含量相对于高危组和低中危组患者水平低(P<0.05),且其拟杆菌含量、肠球菌和大肠埃希菌水平较高危组和低中危组患者水平高(P<0.05),高危组和中低危组患者的肠道双歧杆菌、乳杆菌、拟杆菌含量、肠球菌和大肠埃希菌水平相似(P>0.05).极高危组患者的血清MCP-1 和NLRP-3 水平较高危组和低中危组高(P<0.05),而高危组血清MCP-1 和NLRP-3 水平较低中危组患者高(P<0.05).经过Pearson相关性分析,患者MCP-1 水平与高血压患者的肠道双歧杆菌和乳杆菌含量呈现负相关,与患者的拟杆菌、肠球菌和大肠埃希菌呈现正相关(P<0.05).患者NLRP3 水平与高血压患者的肠道双歧杆菌和乳杆菌含量呈现负相关,与患者的拟杆菌、肠球菌和大肠埃希菌呈现正相关.结论:老年高血压患者的肠道菌群失去平衡,患者血清MCP-1 和NLRP-3 水平与肠道菌群存在相关性.
Relationship between serum MCP-1,NLRP3 inflammasome expression and intestinal flora in elderly patients with hypertension
Objective:To study the relationship between serum monocyte chemoattractant protein-1(MCP-1)and NOD-like receptor protein 3(NLRP3)inflammasome expression and intestinal flora in elderly hypertensive patients.Method:A total of 84 elderly patients with essential hypertension admitted to our hospital from January 2019 to January 2021 were selected.According to the 2018 Euro-pean Hypertension Guidelines,all patients with hypertension were divided into extremely high-risk group(28 cases),high-risk group(38 cases)and low-medium-risk group(18 cases).The in-testinal flora and serum levels of MCP-1 and NLRP3 inflammasome were detected.Results:There were significant differences in blood pressure,serum creatinine and clinical complications among the three groups(P<0.05),and there were no significant differences in the other indicators of the pa-tients(P>0.05).The levels of intestinal bifidobacteria and lactobacilli in the subjects of the ex-tremely high-risk group were lower than those in the high-risk group and low-medium-risk group(P<0.05);and the levels of Bacteroides,Enterococcus and Escherichia coli in the extremely high-risk group were higher than those in the the high-risk group and the low-medium-risk group were high(P<0.05);while the levels of intestinal bifidobacteria,Lactobacillus,Bacteroides,en-terococci and Escherichia coli were similar in the high-risk and low-medium-risk group(P>0.05).The serum levels of MCP-1 and NLRP-3 in the extremely high-risk group were higher than in the high-risk group and the low-medium-risk group(P<0.05),and the serum levels of MCP-1 and NLRP-3 in the high-risk group were higher than those in the low-medium-risk group(P<0.05).Pearson correlation analysis showed that MCP-1 level in patients was negatively correla-ted with intestinal bifidobacteria and Lactobacillus content in patients with hypertension,and was posi-tively correlated with Bacteroides,Enterococcus and Escherichia coli(P<0.05).The level of NL-RP3 was negatively correlated with the content of intestinal bifidobacteria and Lactobacillus in patients with hypertension,and positively correlated with Bacteroides,Enterococcus and Escherichia coli.Con-clusion:The intestinal flora of elderly hypertensive patients is out of balance,and the serum levels of MCP-1 and NLRP-3 are correlated with the intestinal flora.
