A Comparative Study on the Efficacy of Nice Knot and Crossed Kirschner Wire Fixation in Knee Arthroscopy Suture for Avulsion Fracture of Tibial Intercondylar Spine
Objective To compare the curative effect of Nice knot fixation and crossed Kirschner wire fixation in knee arthroscopy suture for avulsion fracture of tibial intercondylar spine. Methods A total of 75 patients with avulsion fracture of tibial intercondylar spine in Huangchuan County People's Hospital from March 2021 to October 2023 were selected as the research objects. They were divided into the control group(n=38)and the experimental group(n=37)according to random number table method.During knee arthroscopy suture treatment, the control group was given crossed Kirschner wire fixation while the experimental group was given Nice knot fixation. Both groups were followed up for 6 months after operation. The indexes related to operation and recovery, such as duration of operation, time to full weight-bearing walking, fracture healing time, and intraoperative blood loss were compared between the two groups.Functions such as knee extension and knee flexion(at the 6-month follow-up)were compared between the two groups. Before operation,at the 3-month and 6-month follow-up,Lysholm Knee Score(LKS)and Visual Analogue Scale(VAS)were applied to evaluate the knee function and pain level in both groups, respectively.Incidence of complications within the 6 months of follow-up were compared between the two groups. Results Compared with the control group, the experimental group exhibited shorter time to full weight-bearing walking, duration of operation and fracture healing time, and less intraoperative blood loss(P<0.05).At the 6-month follow-up,compared with the control group, the experimental group showed higher proportion of patients with normal knee extension and flexion functions, and lower proportion of patients with mild abnormal knee extension and flexion functions(P<0.05).At the 3-month and 6-month follow-up,the scores of LKS in both groups were higher than those before operation, and the LKS scores in the experimental group were higher than those in the control group(P<0.05),while the scores of VAS in both groups were lower than those before operation, and the VAS scores in the experimental group were lower than those in the control group(P<0.05).Within the 6 months of follow-up,there was no significant difference in the total incidence of complications between the two groups(P>0.05).Conclusion In knee arthroscopy suture for patients with avulsion fracture of tibial intercondylar spine, compared with crossed Kirschner wire fixation,Nice knot fixation shows better efficacy in improving the knee extension and flexion functions and knee joint function, relieving the pain, improving the operation related indexes, and promote the recovery of patients with equal safety.
