目的 探讨蜕膜组织肿瘤坏死因子-α(TNF-α)和转化生长因子-β1(TGF-β1)表达与胚胎停育的关系。方法 选取2020年11月至2021年10月就诊于该院妇产科门诊经B超确诊为胚胎停育的患者40例为观察组,另选取同期正常妊娠并要求人工流产的孕妇40例为对照组。采用免疫组织化学法与实时荧光定量逆转录PCR(RT-qPCR)检测两组蜕膜组织TNF-α和TGF-β1及其mRNA表达水平并比较。以两组蜕膜组织中TNF-α和TGF-β1 mRNA相对表达水平的均值为临界值将患者分为不同亚组,进一步分析TNF-α和TGF-β1 mRNA表达水平与胚胎停育的关系。结果 免疫组织化学结果显示,观察组蜕膜组织中TNF-α、TGF-β1阳性表达率分别为80。0%、52。5%,对照组分别为47。5%、75。0%;观察组蜕膜组织中TNF-α表达程度高于对照组,TGF-β1表达程度低于对照组,差异均有统计学意义(Z=-3。622、3。898,P<0。05)。RT-qPCR结果显示,观察组蜕膜组织中TNF-αmRNA相对表达水平高于对照组(6。16±1。25 vs。1。02±0。02),TGF-β1 mRNA相对表达水平低于对照组(0。59±0。19 vs。1。03±0。03),差异均有统计学意义(t=26。144、-14。199,P<0。05);分别以蜕膜组织TNF-α和TGF-β1 mRNA相对表达水平3。59、0。81为临界值进行分析,TNF-α和TGF-β1 mRNA相对表达水平与胚胎停育的发生有关(OR=0。050,95%CI:0。013~0。193,P<0。001;OR=0。125,95%CI:0。055~0。284,P<0。001)。结论 蜕膜组织TNF-α表达升高,TGF-β1表达降低可能与胚胎停育有关。
Expression of TNF-α and TGF-β1 in decidua tissue of patients with embryo damage
Objective To investigate the relationship between the expression of tumor necrosis factor-α (TNF-α) and transforming growth factor-β1 (TGF-β1) in decidual tissue and embryo damage.Methods From November 2020 to October 2021,40 patients attending out-patient clinic of obstetrics and gynecology of the hospital diagnosed with embryo damage by B-ultrasound were selected as the observation group,and 40 nor-mal pregnant women who required artificial abortion during the same period were selected as the control group.Immunohistochemistry and reverse transcription real-time quantitative PCR (RT-qPCR) were used to detect the expression levels of TNF-α and TGF-β1 and their mRNA in decidua tissues,and the expression lev-els were compared between the two groups.The patients were divided into different subgroups with the mean values of TNF-α and TGF-β1 mRNA expression in decidua tissues as critical values,and the relationship be-tween TNF-α and TGF-β1 mRNA expression levels and embryo damage was further analyzed.Results Immu-nohistochemistry results showed that the positive expression rates of TNF-α and TGF-β1 in decidua tissues were 80.0% and 52.5% in the observation group,and 47.5% and 75.0% in the control group,respectively. The expression of TNF-α in decidua tissues of the observation group was stronger than that of the control group,the expression of TGF-β1 in decidua tissues of the observation group was weaker than that of the con-trol group,and the differences were statistically significant (Z=-3.622,3.898,P<0.05).RT-qPCR results showed that the relative expression level of TNF-α mRNA in decidua tissues of the observation group was higher than that of the control group (6.16±1.25 vs. 1.02±0.02),while the relative expression level of TGF-β1 mRNA of the observation group was lower than that of the control group (0.59±0.19 vs. 1.03± 0.03),and the differences were statistically significant (t=26.144,-14.199,P<0.05).The relative expres-sion levels of TNF-α and TGF-β1 mRNA in decidua tissues of the two groups were analyzed with the critical values of 3.59 and 0.81,respectively,which showed that the relative expression levels of TNF-α and TGF-β1 mRNA were related to the occurrence of embryo damage (OR=0.050,95%CI:0.013-0.193,P<0.001;OR=0.125,95%CI:0.055-0.284,P<0.001).Conclusion The increased expression of TNF-α and the de-creased expression of TGF-β1 in decidua might be associated with embryo damage.
