Diagnostic value of fecal differential flora and metabolites combined with diffusion tension imaging in premature infants with white matter injury

目的 探讨粪便16S rRNA基因测序、代谢组学技术联合弥散张量成像技术(DTI)对早产儿脑白质损伤(WMI)的诊断价值.方法 选取2022年4-12月该院收治的出生胎龄<32周且体重<1 500 g的71例早产儿为研究对象,根据MRI检查结果分为 WMI组(n=23)和非 WMI组(n=48).收集出生后24 h内及出生后14、28 d的粪便标本,进行16S rRNA基因测序和LC-MS/MS代谢组学分析,纠正胎龄37~40周时完善头颅MRI+DTI检查,采用受试者工作特征(ROC)曲线分析肠道菌群结构、差异代谢物及DTI联合对WMI的诊断效能.结果 基于属水平诊断WMI的曲线下面积(AUC)为0.88(95%CI:0.74~<1.00),WMI1和非WMI1差异代谢物集诊断 WMI的AUC为0.98(95%CI:0.96~0.99).胼胝体压部的ADC值(r2=0.230 2,P=0.001)、枕叶白质的FA值(r2=0.291 0,P=0.001)与肠道菌群结构最相关.差异性菌群、代谢物联合FA值(侧脑室旁白质和胼胝体压部)诊断WMI的AUC为0.98(95%CI:0.93~<1.00).结论 特征性肠道菌群、差异代谢物和DTI参数可在很大限度上区分WMI和非WMI.
Objective To explore the diagnostic value of fecal 16S rRNA gene sequencing,metabolomics technology combined with diffusion tension imaging(DTI)in premature infants with white matter injury(WMI).Methods A total of 71 premature infants with gestational age<32 weeks and body weight<1 500 g treated in this hospital from April to December 2022 were selected as the research subjects and divided into the WMI group(n=23)and non-WMI group(n=48)according to the MRI results.The fecal samples were col-lected within 24 h after birth,on 14,28 d after birth,and 16S rRNA gene sequencing and LC-MS/MS metabo-lomics analysis were performed.All subjects underwent brain MRI+DTI examination when corrected gesta-tional age was 37-40 weeks.The diagnostic efficiency of the combination of intestinal flora structure,differ-ential metabolites and DTI on WMI was studied by receiver operator characteristic curve(ROC).Results The area under the curve(AUC)for diagnosing WMI based on the generic level was 0.88(95%CI:0.74-<1.00).AUC of WMI1 and non-WMI1 differential metabolite set for diagnosing WMI was 0.98(95%CI:0.96-0.99).The ADC value of the splenium of corpus callosum(r2=0.230 2,P=0.001)and FA value of occipital white matter(r2=0.291 0,P=0.001)were most related to intestinal flora structure.AUC of differential flo-ra and metabolites combined with FA values(lateral ventricle white matter and corpus callosum pressure)in the diagnosis of WMI was 0.98(95%CI:0.93-<1.00).Conclusion Characteristic intestinal flora,differen-tial metabolites and DTI parameters could distinguish WMI from non-WMI to a great extent.
