Research on the Effect of"Hospital-Community-Family"Integrated Management Model on the Health of Elderly Osteoporosis Patients
Objective:To evaluate the effect of the"hospital-community-family"integrated management model on patients with osteoporosis.Methods:Ninety patients with osteoporosis(>60 years old)were recruited from the community and randomly divided into a comprehensive management group and a control group.Patients in the comprehensive management group received comprehensive interventions.The control group did not carry out any other interventions except for hospital treatment.Before the start of follow-up and 6 months after the follow-up,evaluations were conducted on indicators such as bone density,pain level,quality of life(SF-36),and disease cognition.Results:After 6 months intervention,there was no significant change in bone density between the comprehensive management group and the control group,however,pain level and disease cognition significantly improved in both groups(P<0.01),with significantly greater improvement in the comprehensive management group than in the control group.At the same time,multiple dimensions of the SF-36 evaluation in the comprehensive management group,such as health level,physical fitness,emotions,and social function,showed significant improvement(P<0.01),while there was no significant change in the control group.Conclusion:The integrated management model of"hospital-community-family"can effectively improve the pain perception,enhance quality of life,and promotes disease cognition of osteoporosis patients,which provides important practical guidance for comprehensive management of osteoporosis.
