Application of Family Doctor Contract Service Mode in Community Hypertension Control
Objective To observe the effect of adopting a family doctor contract service model on health management of elderly patients with primary hypertension in the community. Methods 200 community hypertensive patients from January 2022 to January 2023 were selected as the subjects,and were divided into a control group and an observation group based on the order of community registration,with 100 cases in each group. The control group received routine guidance,while the observation group received family doctor contracted team services. Compare the satisfaction,blood pressure indicators,and improvement in lifestyle between the two groups. Results There was a significant difference in blood pressure levels between the two groups(P<0.05);The difference in improvement rate of lifestyle in the ob-servation group was significantly better than that in the control group(P<0.05);The satisfaction of the observation group was better than that of the control group,and the difference was statistically significant(P<0.05). Conclusion Implementing family doctor contracted service team cooperation intervention for community hypertension patients can improve their unhealthy lifestyle,stabilize blood pressure levels,and achieve ideal disease control effects.
