基于PMC指数模型的我国分级诊疗政策量化评价研究

分级诊疗制度建设是深化医改的重要内容,是合理配置医疗资源的重要举措.对分级诊疗政策进行量化评价可以评估现有政策的科学性,明确优势与薄弱环节,为今后分级诊疗政策的制定与完善提供决策依据.结合已有研究成果以及分级诊疗政策特点,运用文本挖掘法和专家访谈法,通过构建PMC指数模型对我国2012-2021年颁布的8项分级诊疗政策文本进行量化评价.8项政策的PMC指数平均值为6.85,总体处于良好水平,其中有1项政策为优秀级别,5项政策为良好级别,2项政策为可接受级别,主要存在缺少中长期规划、事前监督重视程度较低、资金投入方面内容较少等问题.因此,应该发挥多元化政策时效的特点,完善政策监督评价体系以及加大分级诊疗的资金投入.
The construction of hierarchical medical system is an important part of deepening healthcare reform and an important measure for better allocation of healthcare resource.The quantitative evaluation of hierarchical medical system can evaluate the scientificity of the existing policy,clarify the advantages and weaknesses,and provide a decision-making basis for the formulation and improvement of healthcare policy in the future.Based on the existing researches and the characteristics of hierarchical medical sys-tem,this paper used text mining and expert interview method to quantitatively evaluate the texts of eight hierarchical medical policies promulgated in China from 2012 to 2021 by constructing a PMC index model.The average PMC index of the policies was 6.85,which was generally at a good level,of which 1 policy was excellent,5 policies were good,and 2 policies were acceptable.The sample policies mainly have the problems of lack of medium and long-term planning,low attention to prior supervision,and obvious insuf-ficient content of capital investment.The authors put forward optimization suggestions for the above prob-lems,including promoting diverse policy effectiveness,improving the policy supervision and evaluation system,and increasing the capital investment in hierarchical medical system.
